Central Cheshire Integrated Care Partnership (CCICP) is looking to expand it's current Advanced Community Practitioner workforce on a permanent basis to support Urgent Community Response and Virtual Ward.
Please note that there are 2 types of role available: Advanced Community Practitioners (Band 8A) Trainee Advanced Community Practitioners (Band 7) Please be aware that a full UK driving license and access to a vehicle that can be used for work purposes are requirements for working in a community setting.
Main duties of the job
Working as an Advanced Community Practitioner at CCICP you will be primarily conducting home visits in community for patients that are acutely unwell and at risk of admission to hospital. You will be working autonomously to assess, diagnose and treat accordingly to the individual patient needs as part of a multidisciplinary community team.
You will be joining an established multi-professional team of Advanced Community Practitioners and work alongside District Nurses, Macmillan Nurses, Complex Case Practitioners and physio/occupational therapists. and are encouraged to work across the four pillars of advanced practice.
You will be supported to work across the non clinical pillars by undertaking quality/safety improvement projects as CCICP consistently strives to remain at the forefront of patient safety and service quality.
We can offer a range of shift patterns across the service which is required to provide the Urgent Community Response service 08:00-20:00 7 days a week, including of bank holidays.

We also pride ourselves in staff development and training with a variety of internal and external courses available to staff, overseen by our Therapy & Nurse professional lead to ensure the ongoing maintenance of high standards within the workforce.
