Clinical Pharmacist Carlisle Primary Care Network

Weekend and late evening shifts may be required

A great opportunity is available for the right person to join an established team of PCN pharmacists and Pharmacy Technicians. In North Cumbria there is a network of PCN pharmacists and pharmacy technicians who have developed the PCN pharmacy team roles and we are now seeking more pharmacists to join the team working in the PCN Carlisle Primary Care Network.

The PCN along with the North Cumbria Integrated Care NHS Trust recognises the value that pharmacists can bring to general practices and to our patients and we look continue to grow our PCN pharmacy team.

Our aim is to provide exemplary patient care through the optimisation of medicines use for all patients including our Care Home residents. Training will be provided as part of this role including access to an approved non-medical prescribing course.

Main duties of the job

PCN Pharmacists can undertake many roles to improve patient care and to optimise the use of medicines including the following:
  • Carry out structured medicines optimisation reviews in care homes
  • Review prescribing against local and national targets and engage with Practice prescribing teams
  • Run clinics where medicines are the main interventions
  • Conduct NHS Health Checks on completion of appropriate training.
  • Conduct care planning
  • Review and act on Pathology results.
  • Monitoring patients with complex long term conditions ensuring that medication is optimised
  • Provide clinical advice and expertise on medication.
  • Work across interfaces of care liaising with other providers i.e. secondary care, mental health and community pharmacy

It is a requirement of the role to be registered with the General Pharmaceutical Council. Primary care experience would be an advantage, however, is not essential. Flexibility, enthusiasm and good clinical knowledge are essential.

Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor other clinical pharmacists and pharmacists with experience of working in a PCN role.

You will need to register and complete a formal education pathway for GP practice based pharmacists such as the Centre of Postgraduate Pharmacy Education course.

Standard Check: �18 + Administration cost of �3.85 = �21.85. Deducted at �6.85 from 1st month salary and �5 deducted from salary over following 3 months.

Enhanced Check: �38 + administration cost of �3.90 = �41.90. Deducted at �11.90 from 1st month salary and �10 deducted from salary over following 3 months.

Person Specification

Knowledge

Essential

  • Broad clinical and pharmaceutical knowledge
  • Relevant specialist knowledge
  • Demonstrable understanding of current issues within pharmacy and the NHS
  • Continuing professional development evidenced by portfolio.


Desirable

  • IT including spreadsheet and database packages


Experience

Essential

  • Significant post-registration experience in hospital pharmacy, community pharmacy or primary care pharmacy.
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.


Qualifications

  • MPharm or equivalent
