An exciting opportunity has come up to work as part of The Assertive Transition Service (ATS) working to expedite the safe, smooth discharge of forensic mental health patients from secure hospital into the community, by providing earlier support in a person's discharge pathway and more robust care and support post-discharge along with the receiving care team. ATS work with Mental illness and Learning disability and autism cohorts.

The ATS cover the East Midlands with 2 teams, the South team is based in Northampton and they cover Northampton and Leicester and the North team are based in Nottingham and cover Nottingham, Derby and Lincoln. The teams consist of multi-disciplinary health and third sector agency and peer support staff working in an alliance to form one integrated service with Rethink and Framework who deliver support work and substance misuse support.

As a Mental Health Practitioner you willhold a core mental health profession (Nursing, Occupational therapy, social work). The post holder will have some experience of working with complex patients in the community and inpatient settings, with knowledge of Secure services and with skills to assess patient's needs, analyse and develop treatment plans within a multidisciplinary framework. There will be an expectation to travel over Notts, Derby and Lincs on a regular basis (travel reimbursed)

Main duties of the job

The role requires an experienced nurse with specialist skills to assess patient's needs, analyse and formulate treatment plans within a multi-disciplinary framework, experience of working within forensic services or with individuals diagnosed with a Learning Disability and/or ASD.

You will travel between the 3 areas working into NHS and private providers, attending relevant meetings (Ward reviews/CPA/117/Professionals meetings) providing the interface between patients clinical teams and community teams supporting the coordination of patients care.

As the Mental Health Practitioner you will assess and collaboratively produce care plans which will inform the substance misuse workers and support workers of the work that is required to ensure a positive transition process into the community.

Job responsibilities

We are looking for an experienced registered nurse (RLDN, RMN) with current registration with the nursing and midwifery Council (NMC).

You will work as part of a multi disciplinary team, carrying a complex community caseload and providing in reach work to secure settings to support the transition of individuals leaving hospital settings that present a forensic risk.

You will have sound knowledge and understanding of working with individuals with a recognised Mental Illness/IDD and/or ASD , with a forensic risk. The importance of working with colleagues from multiple agencies to ensure a robust package of care is available.

You will develop and work in partnerships with Community Forensic Services, Adult Mental Health Services, Private Sector and Local Authorities.

You will work directly with individuals and their families and carer groups.

You will deliver person specific interventions inclusive of training packages

You will have a sound knowledge of Mental health legislation including MHA, MCA including DoLs.

This post is eligible for a Golden Hello payment of �2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust and have not been an employee of the Trust in the previous 3 years. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

Desirable

• Demonstrate working knowledge of Mental Health Act (1983) MCA and DOLS Forensic Sections
