The Head of the Transfer of Care Hub is a senior system appointment to deliver and lead a system-level discharge coordination centre for Oxfordshire health and social care system on behalf of Oxford University Hospitals NHS FT (OUH), Oxford Health NHS FT (OH) and Oxfordshire County Council (OCC).
As a highly experienced, credible, senior professional/clinician you will work in partnership with the system health and care leaders to coordinate care and support to reduce the number of days away from home for our patients. A hugely rewarding role with scope to work collaboratively to make a real difference to our patients.
The post-holder will use their knowledge and authority to manage effective use of discharge pathways across the system and leading system wide change in practice and culture.
Main duties of the job
Key Success Criteria
Delivery of an effective and multi-agency approach to transfer of care from a hospital setting to home for people leaving hospital who may require ongoing health or social care input (supporting discharge pathways 1, 2 and 3). Demonstrable reduction in the overall use of bed provision both within the acute hospitals and Community wards and discharge to assess / interim bed provision within the community. Contribution to a leadership culture of collaboration and trust, based on honest conversations and an understanding of system wide factors that may be impacted by the effectiveness of the Transfer of Care Hub. Local compliance with national policies and guidance regarding hospital discharge.
By demonstrating exceptional leadership and communication skills the successful post holder will proactively drive system collaboration within a high risk and highly complex environment. They will provide operational leadership and management to teams across, OUH, OH and OCC whilst demonstrating mental agility and resilience in responding to highly sensitive situations necessitating rapid shifts between strategic and operational decision making.

Lead delivery of multidisciplinary operational meetings by the Transfer of Care Hub to support flow out of acute hospitals and community beds and ensure timely follow up of agreed community assessments.
Provide operational leadership and management responsibilities across teams for OUH, OH and OCC which support effective discharges for the Oxfordshire system.
Operate as a service expert for discharge planning, co-ordination and outcomes by providing expert advice to managers, matrons, professionals, other staff and partners.
Work with colleagues across the system to develop and deliver an outcome framework to measure success of the Transfer of Care Hub against key national and locally agreed criteria.
Conduct evaluation of the Transfer of Care Hub, benchmarking against best practice and policy both locally and nationally to support evidence-based recommendations for the future model of service and ongoing system development.
Provide operational leadership and line management for the Discharge Team and the Short Stay Hub Bed team within OUH and the Patient Flow Team within OH.
