The post holder will be a valued member of the governance team for maternity and neonatal services. With the ultimate focus being on patient safety the post holder will be key in managing incidents and near misses to ensure that they are reviewed timely, so that learning is shared throughout the directorate. As a qualified midwife the post holder will be required to be extremely organised and work within the team to ensure that the governance and assurance requirements are met for the directorate, division and external agencies. The post requires effective communication and an ability to coordinate several different work streams whilst working with all members of the wider directorate multi-disciplinary team. The post holder will remain objective to ensure that incidents and near misses are reviewed in line with local and national guidance to ensure lessons learned support staff in the delivery of evidence-based, woman centred care.

Main duties of the job

Governance and Assurance Midwife

To assist the Governance and Assurance Managers for Maternity and Neonatal with governance, assurance and compliance workload. This will include

Approving, managing and closing DATIX incidents.
  • To assist with the management of incidents
  • To contribute and assist in completing gap analysis and action plans.
  • To assist with any cases which meet the criteria for external reviews
  • To be involved in collating information needed by the senior directorate management team for example dashboard data.
  • To be actively involved in the patient safety incident response framework (PSIRF) implementation.
  • To assist in sharing lessons learned to the wider directorate
  • To assist with Duty of Candour requirements.
  • Work within the team and contribute to updating the risk registers.

PMRT Lead Midwife
  • To assist with the completion of the Perinatal Mortality Review Tool (PMRT) for all eligible stillbirths and neonatal deaths.
  • To contribute to collating the papers for a multi-disciplinary review of PMRT cases and proceed to finally publishing those cases thereafter if needed.
  • To assist the governance team in collating and monitoring issues which arise from all PMRT cases (stillbirths and neonatal deaths) until the agreed actions have been implemented.

Clinical

To be responsible for the assessment, planning, implementing and evaluations of Midwifery care for mothers and babies.

Obtaining womens informed consent throughout the childbirth continuum

To provide support and care for the woman/baby and her family respecting their need for privacy and dignity

Establish and maintain effective communication with individuals and groups about difficult or complex matters, overcoming any problems in communication.

Develop and maintain Midwifery knowledge and practice by attending and participating in continuing professional development and contribute to the development of others.

Monitor and maintain health, safety and security of self and others in own work area.

Contribute to the development of services.

Ensure own actions promote quality and alert others to quality issues. Support peoples equality, diversity and rights.

Assess peoples health and wellbeing needs when those needs are complex and change across the caseload.

Plan, deliver and evaluate care pathways to address womans complex and changing health and wellbeing needs.

Monitor the effectiveness of care delivery and renegotiate the programmes of care where indicated to meet the womans need.

Utilise highly developed physical skills where appropriate.

Support patients/carers, encouraging them to promote their own health and wellbeing and to express their interests and concerns.

Communicate with a wide range of people to explore complex issues and make complex decisions.

Effectively communicate complex and sensitive information.

Competently receive sensitive information concerning womans care pathway and provide information using persuasive, reassuring skills as required, e.g. Breaking bad news/special needs/dealing with anxious relatives.

To liaise with the multi-disciplinary team, coordinating and participating in case discussion as required

Agree the arrangements for communication with the woman/family/carers, and document these in accordance with Trust policy and the patients right to confidentiality.

Ensure that essential information on the patients condition and progress is recorded by self and team members appropriately according to NMC guidelines.

Inspire others and encourage them to seek advice and solutions to problems.

Challenge other to take an active part in developing knowledge, ideas and work practice.

Embrace the public health agenda and support the delivery of local delivery plans e.g. breastfeeding, smoking cessation, teenage pregnancy, midwifery led care.

Participate in teaching of parent education.

Understand and undertake data collection effectively using the agreed systems.

Ensure own actions promote quality and identify and manage any risks.

Keep up to date and act consistently with quality standards and guidelines within their own clinical area and associated areas.

Have an understanding of the quality agenda and how standards of care can be maintained.

Monitor the quality of work in own area, raise quality issues and related risks and bring to the attention of others.

Take immediate and appropriate action in relation to adverse incident reporting utilizing the hospital incident reporting system.
