Lincolnshire Community Heath Service (LCHS) have an opportunity for a Registered Nurse to join their Lincoln North Community Nursing Team.

We have a full time vacancy for Band 5 Community Staff Nurse.

As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence based practice.

Main duties of the job

Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery.

The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patients care is effectively co-ordinated, demonstrates a positive impact on our patients.

Job description

Job responsibilities

As this is a community post, you must be able to travel to patients homes and clinic settings. Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential.

The service works through the hours of 8:00am to 6:30pm 7 days a week and is covered by flexible working patterns.

The Community Nursing service delivers a 12 week induction programme for all new staff. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential.

Qualifications

Essential

  • Ability to demonstrate current 1st level UK NMC adult nurse registration
  • Evidence of continuous professional development
  • Demonstration of mentorship, supervisor or assessor qualification and its use in practice


Desirable

  • Evidence of completed leadership modules/qualification
  • Evidence of V100 or V150 Community Practitioner prescribing course undertaken


Experience

Essential

  • Can demonstrate experience of assessment, care planning and care delivery
  • Experience of delegation and supervision of others
  • Evidence of the application of research and evidence based care
  • Evidence of experience in teaching / disseminating knowledge and skills
  • Experience of team working


Desirable

  • Experience of health promotion/ coaching strategies

Essential

  • Demonstrates a range of clinical skills pertinent to area of practice
