LNWUH Nhs Trust has 3 Critical Care Units comprising of 40 beds across 3 sites. The 3 Units admit in excess of 2200 patients per year. Specialities include Maxillo facial surgery, Complex Colorectal, Renal , general Medical and Surgical patients , along with acute patients form our PCI service and Accident and Emergency department.

We are looking for a Senior Sister / Charge Nurse, who will support our Critical Care both at The Ealing Hospital Base site, and supporting Critical Care across site, when required.

Candidates must embody and demonstrate the Trusts values, uphold the responsibilities of the job description and the person specification. Candidates should be able to demonstrate leadership skills and Advanced intensive Care skills in line with a Senior Staff / Team leader position.

Clinical:

Coordinate the care of critically ill patients through the critical care pathway to ensure the effective management of their patient group.

A resource for staff, advising on national, local and Trust policy and procedures/guidelines, ensuring that clinical governance is embedded in practice.

Responsible for identifying and assessing complex clinical and non-clinical issues, implementing resolutions and monitoring/auditing outcomes.

Ensure optimum patient management including patient monitoring equipment management and defined expanded clinical roles.

JOB SUMMARY

A high profile, accessible and responsive leader with clinical and service responsibility for a defined patient group. The Post holder will also be responsible for the development for a team of nursing staff. The person must be able to make informed decisions within their own area and scope of professional practice, involving patients and public to ensure agreed standards, objectives and targets are met. The leader should be able to provide exceptional treatment and care to patients throughout their critical illness.

The practitioner will be able to make informed decisions within the speciality and scope of professional practice. They will work collaboratively within the critical care teams to meet the needs of patients and play a key leadership role in service delivery and adherence to policy and procedures. They will demonstrate safe clinical decision making and expert care for their patient group and provide appropriate plans of care.

The post holder will be required to ensure evidence based care is delivered to patients and be involved in audit of practice, research and service improvement.

Promote and deliver evidence based care for patients

Recognition and escalation of the deteriorating patient.

Education and supporting patients with care of tracheostomy.

Education and training provision of identified staff groups, patients and carers within defined areas.

Responsible for accurate, timely documentation of clinical plan in patients records both computerised and paper based.

Utilise and ensure others utilise information and IT systems to secure accurate and timely patient, workforce and resource data available.
  • Identify and implement, in collaboration with the multi-professional team, appropriate areas for research and audit.
  • Implement and support the introduction of new treatment methods relevant to the speciality
  • Deputise for the Clinical Nurse Manager and the Matron in their absence.
  • Establish and maintain professional networking relationships between departments in the trust and attend external meetings to promote best practice in the trust.
  • Support patients, carers and staff during difficult situations arising in the clinical area, e.g. breaking bad news or following an unexpected event.
  • Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.

Communication:
  • Use the SBAR tool to communicate clinical concerns.
  • Maintain effective communication within the team, multi-disciplinary teams and other departments.
  • Communicate complex information effectively to ward staff, multi-disciplinary team, patients and carers.
  • Recognise the need for alternative methods of communication to overcome different level of understanding, cultures and barriers to communication.

Delivery of Quality Service:
  • Prioritise, organise and manage own workload to maintain and promote quality.
  • Deliver care according to local and national guidelines to ensure delivery of evidence based care.
