Exciting opportunity to work within the WSH Community Diabetes Team supporting patients with diabetes out in GP Practices. Opportunity to interface with primary care colleagues and build working relationships for the benefit of our patients. Requires a level of self-direction and autonomy to manage a specific caseload. Scope for both professional and service development to enhance this integration.

Main duties of the job

Candidate will run Nurse Led Clinics out in GP Practices managing patients with diabetes. Follow up of these patients will be via email, telephone and teams contact in between Clinic reviews. Ability to work autonomously with an organised approach is essential along with effective communication skills both with patients and HCP's across primary care.

STAFF CAR PARKING

Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 18:30.

For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.

This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete on call duties within their role.

Person Specification

Education and qualifications

Essential

  • NMC Registration Level 1 Registered Nurse with 5 years post reg experience
  • Demonstration of personal and professional development


Desirable

  • A recognised Diabetes Course and experience working within diabetes
  • Non-medical Independent Prescriber
  • Evidence of academic pathway towards a masters degree


Experience and knowledge

Essential

  • Evidence of continuing professional development
  • Experience in multidisciplinary working
  • Experience of developing, training and education in clinical practice
  • Able to demonstrate a wide knowledge of current trends in nursing and application to practice


Desirable

  • Evidence of audit experience
  • Proven experience within diabetes specific role at Band 6
  • Experience of implementing Service Improvement

  • Good oral and written communication skills with people from a wide variety of backgrounds
  • Ability to impart complex information to patients, relatives and carers, with different levels of understanding
