An exciting opportunity has arisen for a Registered Nurse to join our Specialist Palliative Care team.

The Specialist Palliative Care team compromises of Clinical Nurse Specialists and Specialist Doctors. The specialist nurses work throughout the adult wards supporting patients with specialist palliative care needs (regardless of diagnosis) by addressing symptom control and offering psychological support to patients and their families.

As a specialist team the Clinical Nurse Specialists you will be involved with clinical work, audit, education, delivery of new end of life and palliative initiatives and contribute to local policies and guidelines.

This role is 30 hours per week: 21 hours is permanent and 9 hours is on a fixed term contract/secondment until 31st May 2026.

The post holder will be required to work 9 - 5pm and including weekend working.

Main duties of the job

As post holder your duties will include:
  • The opportunity to support all ward staff to deliver palliative and end of life care
  • To mentor staff to deliver excellent palliative and end of life care
  • To manage a clinical caseload
  • Offer psychological support at level 2 for all patients and their families
  • To participate in audit and where appropriate research and develop reports and actions plans
  • The opportunity to be able to deliver education to post and pre- registration healthcare professionals re: specialist palliative care
  • Being able to contribute towards the development of policies, protocols and guidelines related to palliative and end of life care as required
  • To be pro-active in service development
  • The opportunity for professional development as an autonomous practitioner in a specialist nurse/educator role

Job description

Job responsibilities

  • To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments.
  • To work closely with medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
  • To ensure that patients are seen within recommended time scales and targets
  • To visit inpatients as appropriate to ensure continuity of care and to give advice/ support as required. Ensuring full and thorough documentation of all clinical actions and communication to the wider MDT
  • To accept referrals from medical/nursing/MDT staff both within and outside the Trust for patients who meet the referral criteria for SPC
  • To periodically review caseload and to discharge patients appropriately within team guidelines
  • To communicate sensitive condition related information effectively with patients and carers in a timely and understandable manner, using different communication methods to clarify understanding as required.
  • To liaise with other Specialists Nurses, Consultants, GPs, Allied Healthcare Professionals, other members of the MDT, voluntary agencies, National charities, other agencies within and outside the Trust.
  • To provide accurate written information following patient consultation to GPs and other members of the MDT as required
  • To ensure the timely completion of summary care records for patients being discharged to their usual place of residence and to ensure the completion of IT systems with relevant clinical information
  • To work as an autonomous practitioner responsible for managing and prioritising own workload against the needs of patients and service requirements
  • To assist the Line Manager in the provision of clinical and managerial leadership and support of all nursing and clerical staff in the Trust and SPC team
  • To deputise for senior colleagues in their absence as required

Person Specification

Qualifications

Essential

  • Registered Nurse on the appropriate part of the NMC Register
  • To have completed Mentor Preparation Programme or demonstrate mentorship experience if applicable to clinical area
  • Post Registration qualification and or experience relevant to the speciality


Experience

  • Significant post-registration experience including some in a position relevant to the speciality in the last 24 months
