A very rare and exciting secondment opportunity, covering maternity leave has arisen to join the North West London Urgent Community Response Single Point of Access. Bringing together eight different Urgent Community Response services across eight boroughs to provide a swift and reliable clinical advice and referrals service to our London Ambulance (LAS) and NHS 111 colleagues.

As a part of our team you will undertake a supportive triage role for referrals coming from 111, LAS crews with patients and the LAS Clinical Hubs.

Please note, although this role is not directly patient facing, you may be required on occasion to spend time working clinically as part of one of the eight Urgent Community Response services. This would most likely be in Ealing. We actively encourage hybrid working where possible (and at management discretion).

Main duties of the job

  • Provide the highest level of clinical leadership to referrers and team members.
  • Offer excellent customer service and ensuring that the most appropriate clinical outcome is reached.
  • Be a point of clinical advice and guidance for callers from NHS 111 / LAS Crews or Clinical Hub
  • Approach challenges with a can-do attitude, proactively managing difficult clinical queries, appropriately signposting or aiding in creating a suitable management plan.
  • Be involved in building relationships with sister-services and stakeholders
  • Be prepared and open for changes to the service and willing to participate in research / pilot schemes.

You will additionally be expected to be able to (Although some of these may not form a regular part of your role):
  • Attend patients to do an Urgent Community Response initial assessment and for follow up visits.
  • Develop your clinical skills into a more well-rounded holistic approach to patient care.
  • Develop new skills outside of your normal scope of practice including skills from Paramedicine, Nursing, Physiotherapy and Occupational therapy.
  • Supervise team members and students
  • Attend in-service training and have Continued Professional Development opportunities
  • The NWL UCR SPA works 7 days a week from 8:00am until 20:00. You will be expected to work weekends and bank holidays although these will be distributed throughout the team. A shift consists of 12-hour day, including dynamic rest periods.

Essential

  • Clinical Reasoning In Physical Assessment course or equivalent demonstratable experience
  • Registered: Nurse / Paramedic / Physiotherapist / Occupational Therapist / Pharmacist


Experience

  • Post registration experience in a UCR / Rapid Response environment within an MDT
  • Experience of undertaking assessments in the community with long term conditions
  • Able to give advice and signposting to stakeholders across a wide range of knowledge, i.e doctors / paramedics / nurses / carers etc.


Knowledge

Essential

  • Complex clinical decisions including a sound working knowledge of capacity and consent.
  • Knowledge of clinical governance processes
  • Excellent understanding across a range of patient conditions, including frailty, medical, surgical, cardiact, acute on chronic etc


Skills

Essential

  • Advanced clinical decision making skills
  • Able to communicate highly complex information with an excellent telephone manner
  • Able to manage difficult situations and conflicting opinions
  • Able to adapt to the changing needs of the service
  • A good aptitude for IT skills including MS Excel, Outlook, Ability to use telephony software
