The successful candidate will work within the PROSPECT Lead Provider Collaborative based across Cheshire and Merseyside and support the commissioning and Case Management team in delivering a high-quality service within the adult secure care mental illness, learning disability and autism and personality disorder pathway.The role is responsible for maintaining oversight of a specific caseload of individuals within specialised secure hospital environments ensuring access to appropriate treatment, commissioning oversight, quality and governance and discharge planning development.You will work closely and collaboratively with providers within the PROSPECT partnership as well as other provider organisations and commissioning team members in Local Area Teams, to proactively support the pathway.You will be accountable and responsible to the Senior Pathways Manager and the Head of Quality who will support you with your day-to-day responsibilities.You will provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
Main duties of the job
As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from their LAT in specialised mental health care settings, within the NHS and Independent sectors.
To provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
The role supports the business in driving transformation as well as value for money in planning, commissioning and service. The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
To provide clinical assurance of the quality of the care provided by specialised services within the LAT. Improving quality and outcomes Supporting the team in the assurance of specialised services compliance to applicable specification and guidance. The post-holder will provide the link with Clinical Commissioning Groups and their commissioned services within the LAT to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages. Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved To take action in respect of Safeguarding Policies and Procedures as required

- Registered Mental Health/ Learning Disability Nurse or other registered health or social care professional
Desirable
- Possess a relevant additional professional qualification e.g., Forensic Psychiatry Diploma, Master's level, or equivalent level of experience of working at a senior level in specialist area
Knowledge

- Detailed knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation
- Detailed operational knowledge of secure services, including risk assessment/management
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent
