As part of our continuous quality improvement initiatives the Adult Community Mental Health Services have created new mental health practitioner posts specialising in undertaking brief initial assessments and providing advice and information to those contacting our services.

This is an excellent opportunity for experienced clinicians to utilise their knowledge and skills in supporting service users and their carers in accessing the most appropriate services for their needs

The post holder will work as part of the Adult Community Mental health Services embedded within the Trust Single Point of Access to assess the needs of all those referred to the Adult secondary care pathway.

An excellent understanding of the issues people with mental health needs may experience is essential for this post as is a good understanding of the range of support available - ranging from on line and digital self help resources, community based services and health and social care pathways , including Safeguarding.
