To provide a comprehensive city wide Community Cardiac services focusing predominately on heart failure but also supporting with cardiac rehabilitation and for patients referred to the service from the Birmingham health economy. The service is for patients aged 18 and above, and will be delivered in a variety of locations across Birmingham Community Healthcare NHS Trust (BCHC).
The post holder will assist in shaping the future delivery of services across the Trust in order to support the operational developments of the service. Working as an autonomous practitioner, they will prioritise their own work load and be responsible for leading the team and delegating duties to all team members working alongside the other Cardiac Services Team Leaders.

Utilise advanced clinical skills to ensure patients and carers receive the best possible care and outcomes through clinically effective practice across the community nursing services.

Work in accordance with NMC Code of Professional Conduct
Act as a role model in expert practice facilitating and supporting the delivery of effective patient care.
Support and assist staff in developing appropriate management plans of care for individual patients or client groups.
Identify and promote areas for practice development, including guidelines and procedures to support the delivery of care, ensuring that all care is evidence based.
Facilitate and participate in Trust wide groups that require specialist nurse input.
To provide appropriate and consistent professional advice to other senior members of the team, acting as a point of contact in times of difficulty and when support is required.
Facilitate effective communication between primary, secondary, community, social care and voluntary services in order to promote best possible outcomes.
Provide regular reports to the Service Manager to allow for regular monitoring and evaluation of the service.
The post holder will have a high degree of autonomy within the role as a clinical specialist utilizing their knowledge, skills and experience. They will be required to practice within the professional scope of the role.
Participate in clinical supervision and recognize when it may be necessary to access advice and support from senior colleagues and consultants.
The post holder will be IT literate with all systems including RiO or equivalent data bases, outlook, power point, access, publisher, word, intranet and internet.

Qualifications & Training
Essential
- Current NMC Level 1 Registration
- Specialist Cardiac qualification, i.e. Glasgow Caledonian
- Educated to degree level or evidence of equivalent experience
- Evidence of post graduate study in cardiac care, e.g. ECG course
- Specialist teaching qualification i.e. ENB 998 or equivalent
- Basic Life Support (BLS) trained
- Independent & Supplementary Nurse Prescriber
Experience

- Substantial experience of managing a team of multi - disciplinary staff.
- Substantial experience of working within a complex cardiac environment
- Substantial experience in delivering community based cardiac care
- Evidence of delivering formal / informal education and teach self management to complex groups of patients
- Experience of managing complaints informally and through the formal Trust route
- Experience and understanding of NHS and health care systems
- Understanding of the needs of socially and culturally excluded groups
- Experience of organising and chairing team meetings and planning staff events
- Experience with finance, budget management, effective use of resources, procurement and the use of SBS systems
- Significant experience of managing data bases, producing and analysing reports for audit.
