The core purpose of this role is to work across the older peoples' inpatient mental health service to embed the Occupational Therapy assessment and intervention pathway across the service within the framework of MOHO, in collaboration with the wider MDT staff.

The post holder will function as part of the MDT to deliver the Interventions required to support individuals through their recovery pathway and equip them with the skills and confidence to enable a safe discharge from the service and to support assessments and decision making within the MDT.

The post holder will be highly skilled and competent in conducting OT assessments developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures.

The post holder will provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.

The OT will be responsible for developing treatment plans and intervention to be delivered by the team's Technical Instructor and Activity Co-ordinator.

Main duties of the job

o To be responsible for the assessment treatment and evaluation of patients referred specifically for OT intervention from the inpatient service.o To be able to work across all functions within the inpatient service and support patients to achieve their individually agreed goals.o To work as part of a multi-disciplinary team to, assess, plan and implement individualised programmes of care/interventions in accordance with CPA (where appropriate).

o To assess OT need and plan therapeutic OT interventions using the Model of Human Occupation.o To lead on the development of group based interventions to aid recovery and promote social inclusion.o To facilitate the development of peer support networks as part of the discharge pathway from the service.

Education / Qualifications

Essential

• Must hold a diploma or degree in Occupational Therapy and be registered with HCPC.
• Evidence of post qualification experience and continuing professional development to post graduate diploma level.
• Willingness to undertake further training relevant to the role resulting from legislative or service changes, as appropriate.

Experience

Essential

• Substantial post registration experience working in a mental health service.
• Experience in working with older people with severe and complex mental health needs.
• Experience of MDT working.

Knowledge

• Extensive knowledge / experience in social inclusion and recovery model of care.

• Good working knowledge of assessment, care planning, implementation and evaluation processes.
• Comprehensive knowledge of mental health conditions and how they impact on an individual's wellbeing and occupational functioning.
