The Abdominal Medicine and Surgery Clinical Service Unit are looking to appoint a Liver transplant co-ordinator to join the existing team in providing care for patients undergoing transplant assessment, waiting list management and participate in a 1:5 oncall rota.

The successful candidate will work closely with other members of the MDT and the team will ensure that you receive the support you need to develop your potential in this post.

Main duties of the job

The post holder will have a key role in the development and delivery of a specialised service for transplantation across LTHT.

The post holder will be responsible for ensuring effective interdisciplinary and multidisciplinary partnership working with key services. They will play an integral role in order to develop the strategic provision of this service. They will be responsible for the development of a clinical pathway and the overall journey of the patient ensuring that patients are managed in accordance with national standards and guidelines.

The post holder will be responsible for managing a caseload of patients and the assessment process, referring patients appropriately to other healthcare professionals, medical, surgical, psychological and social.

They will provide and manage an expert specialist service, developing an advanced level of professional autonomy and accountability in the provision of care acting as an ambassador for organ donation and transplantation

They will participate in a 24/7, 365 days a year on call rota, receiving referrals of organs offered for transplantation from the UK and Europe taking into account the donor aetiology and clinical status with the consultant on call to ensure safe, efficient and seamless co-ordination of transplant surgery

Job responsibilities

Work within the Transplant Co-ordinator team to develop and support our service for patients who require transplantation within the Leeds Teaching Hospitals

Work as a highly specialised nurse/practitioner, will work within the transplant Co-ordinator team using evidence based practice to develop objectives and standards of clinical practice to support the delivery of the transplant service

Continuously monitor services provided to ensure that the highest professional standards are maintained and audited.

Analyse data and present the findings to the relevant personnel, locally, nationally and internationally to promote the sharing of knowledge and practice

Ensure the most efficient and effective use of resources available, implementing initiatives to ensure a cost effective service

Lead on, participate in and support others in the application of quality initiatives with the aim of streamlining adult and paediatric transplantation for patients ensuring their assessment through to transplant is efficient

As a team of highly specialised nurses and practitioners, participate in analysing existing service delivery and determine future service needs in line with local and national requirements

Qualifications

Essential

  • Registered Nurse (Level 1 or 2) / Registered Allied Health Professional
  • Relevant professional degree
  • A relevant specialist accredited post graduate course to masters level or equivalent
  • Recognised teaching and assessing qualification
