The Centre for Spinal Studies and Surgery (CSSS) is based at Queen's Medical Centre of the Nottingham University Hospitals, one of the largest university hospitals in Europe. The CSSS has been recognised as a Centre for Spinal Excellence within Europe in December, 2023. The core of the CSSS is formed by the spinal ward D8, the spinaltheatresand the spinaloutpatientdepartment.
As a team, the CSSS provides expert spinal care even for the more rare and complex pathologies. Being a recognised national and international referral centre for complex spinal pathologies.
The vision for the Spinal Case Manager at the Nottingham Centre for Spinal Studies and Surgery is to create a seamless, patient-centered care pathway that exemplifies clinical excellence and operational efficiency. This role is dedicated to ensuring that every spinal patient receives timely, expert care tailored to their individual needs, from initial referral through rehabilitation and eventual transfer to local hospitals or discharge. By fostering strong leadership, advocating for patients, and driving continuous improvement, the Spinal Case Manager will play a pivotal role in enhancing patient outcomes and experiences, reinforcing our commitment to delivering excellence in all we do. This position will not only support the multi-professional team but also spearhead innovative practices and research initiatives, positioning our center as a leader in spinal care across the East Midlands.
Main duties of the job
Working with a vision of delivering excellence in all we do the post holder will focus on quality and access in the co-ordination of the Spinal patient pathway. Specifically the post holder will be responsible for supporting the speciality leadership team to provide senior professional, operational and clinical leadership to:
Promote clinical excellence in the care of patients along the spinal patient pathway
Provide strong visible leadership along the pathway of the patient
Provide expert advice and support to the Multi-professional team along the patients pathway
Ensure that spinal patients are managed in the timeliest fashion according to their needs working closely with the Divisional and Trust operational teams
Drive through changes and be involved in audit to ensure that clinical outcomes and patient experience improve continually
Be an advocate and visible point of contact for patients and their families
Contribute to ongoing service improvements
Undertake research and lead audit in own specialist area

KEY JOB RESPONSIBILITIES
Clinical/Leadership:
Support in promoting clinical excellence in the care of patients along the spinal pathway, providing expert support and advice to the Multi-professional team along the pathway enabling the coordination of a seamless service for spinal patients.
Support in providing strong visible clinical leadership skills energising nurses and other clinical staff in pursuit of a shared vision to ensure the effectiveness and safe delivery of care to patients along the spinal pathway.
Support the decision-making process pro-actively, collecting and analysing information in order to make effective decisions in complex situations.
Act as a role model of expert practice across the spinal pathway.
Be a visible point of contact for patients, visitors, relatives and staff, acting as a resource.
Coordinate repatriations and discharge planning for spinal patients, making early referrals to other hospitals for patients who will need inpatient care beyond NUH and actively ensuring any delays are escalated at the earliest opportunity.
Co-ordinate transfers of these patients ensuring appropriate information is communicated to accepting wards and comprehensive documentation is sent with the patient.
Service Delivery:
Contribute to the implementation and co-ordination of a standardised Multi-professional approach to spinal care throughout the patient pathway.
Utilise relevant data to support the development and implementation of policies and protocols and monitor adherence to them.
Contribute to the development of service development strategies, anticipating future needs, to ensure the Spinal Unit meets its commitments on all service delivery and performance targets.
Challenge existing practices, ensuring that progressive solutions, which take into account models of best practice and best use of resources are incorporated into service plans.
Work across organisational and professional boundaries to help implement new solutions to facilitate a smooth patient journey.
Work closely with Clinical Managers, clinicians and support services in other parts of the Trust to ensure delivery plans are compatible and maximise the opportunities for more efficient ways of working.
Performance Management:
Work with the Spinal Head of Service to regularly review the effectiveness of care pathways and to identify and rectify any blockages to performance.
Contribute to the Spinal Units performance in order to build a sustainable future.
Enhance the performance and capability of others through formal and informal interactions.
Support the Departmental leaders to motivate staff to higher levels of performance by increasing their commitment to the Trusts objectives, clarifying the vision and developing effective team working.
Support the speciality to initiate and implement change effectively in response to clinical organisational and National demands.
Governance and Risk:
Contribute actively to the Governance and Risk Agenda of the Spinal Unit.
Support in the development and implementation of effective systems for recording and monitoring governance and risk information.
Help promote a culture where governance and risk management are seen to be everyones responsibility.
Communication:
Establish effective communication networks and strategies within the network and nationally.
Establish effective and seamless communication throughout the patients journey including wards, theatres, radiology and critical care.
Establish excellent communication with wider Multi-professional team and managers in the Trust and wider healthcare community to ensure that services are integrated.
Work closely with patient advice liaison service (PALS) to ensure that the concerns of patients and their families are dealt with quickly and effectively.
Be pro-active in promoting the Trusts vision for delivering excellence in all we do.
Ensure that good practice is rapidly shared within the Spinal Unit and wider organisation where appropriate.
Act as a point of contact for relatives and carers during their hospital stay.
Provide support for patients and their families as appropriate.
Ensure that the Head of Service / Matron are kept informed of Spinal activity and potential problems that may have been identified.
Participate actively in spinal-related meetings
Professional:
Take responsibility for own professional development. Identify personal objectives, reflect on
progress and set a personal development plan annually,
Ensure own practice is kept updated, using an acceptable model of clinical supervision.
Write an annual report on the role which evaluates the service provided.
Take every opportunity to expand own practice in line with principles and guidelines published
by the Nursing Midwifery Council (NMC).
In undertaking this role the post holder must comply with the current NMC Professional Code of Conduct at all times.
Education and Teaching:
Contribute to teaching programme for the Multi-professional team along the spinal pathway, in a variety of formats, recognising the diverse needs of the workforce.
Prioritise own time to be available to coach and mentor others.
Research and Audit:
Ensure that opportunities for non-medical research and audit are maximised to support the improvement of clinical outcomes and patient experience alongside the Speciality team.
Help in ensuring that research and audit activity is conducted in line with Trust standards and that results are shared with other Specialties as appropriate.
