An exciting opportunity has arisen to work within Boston Crisis Resolution and Home Treatment Team. This post would suit a newly qualfied nurse, OT or Social Worker looking to carry out their perceptorship or a more experienced band 5 Clinician looking to develop into a band 6.

The role is to cover the large service catchment area, therefore a full manual driving licence is essential. You will not be expected to use your own vehicle for work use, the team have access to pool cars. The service runs over a 24/7 period, so you will work long days across 7 days a week. Once you have developed to Band 6, you will be required to work night shifts. The team provide assessment to those experiencing acute mental illness and, if indicated, short term home treatment in a suitable setting. The team refer in to a number of longer term services if required, this would be identified by the post holder.

The post holder will provide professional advice to service users and carers as well as support to other agencies and members of the Multi-disciplinary team. You will be expected to have good documentational skills and be able to confidently use the Clinical Information system- RIO. In order to manage risk individuals will be Breakaway trained.

Main duties of the job

oAlways act in a dignified and responsible manner with service users, carers and colleagues, using appropriate language and communication skills, which acknowledge barriers to understanding, cultural differences and diversity.

oTo provide supportive and sensitive communication, written and verbal advice to patients, carers and staff, whilst demonstrating an understanding of the nature and effects of some information and of barriers to effective communication e .g, sensory or cognitive impairment and unrealistic expectations whilst being considerate of client confidentiality.

oDevelop and maintain communication (sensitive, complex and contentious) with other teams, external agencies, in order to promote good client care and deliver initiatives such as Care Programme Approach.

oTo confidently convey complex written and verbal information on both clinical and non-clinical issues and to support the development of these skills with individuals within the team.

oProvide and receive complex information where persuasive, motivational, negotiating, empathic and re-assurance skills are required to support service users through their recovery period.

oMaintain strict confidentiality of complex and sensitive client information at all times.

oDevelop and complete risk assessments, analysing and deciding the most appropriate approach to implementing care plans, agreeing objectives with service users and carers.

Job description

Job responsibilities

  • To provide a high standard of clinical care ensuring safe and effective assessment, treatment, and comprehensive discharge planning.
  • To work flexibly across a 7-day-a-week service, working 9am to 10pm shifts
  • Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances and enable them to understand, manage and where appropriate change their behaviour.
  • Plan, implement, review, and improve interventions to meet peoples identified needs and manage their inherent risk.
  • To assess carers and familys needs and develop, implement and review programmes of support for cares and families.
  • To identify & protect people from abuse, neglect and harm working alongside safeguarding.
  • Enable individuals to develop independent living skills and live in the community.
  • To work collaboratively and promote effective working relationships with members of the team, ensuring effective and appropriate clinical decision-making, both within the Crisis Team and with external agencies.
  • To maintain accurate and timely clinical records.
  • To adhere to N.M.C. codes of professional conduct and ethics, plus associated legislation.
  • To develop clinical practice having due regard to guidelines of the N.S.F. for Older People, N.S.F for Mental Health, N.I.C.E. and N.I.M.H.E.
  • To demonstrate empowering leadership skills within the Crisis Team and seek opportunities in local and national area as to promote and develop the profession.
  • To participate in Clinical Governance activities, including: induction, supervision, personal development review, health and safety, risk management and audit.
  • Develop effective and supportive links with other health and social care staff. To create networks that improve the pathway of care for clients referred to the Crisis Team.


Person Specification

Qulifications

Essential

  • RVN or RNLD or RN: 1 st Level Registration (NMC) or degree or equivalent. / Allied Health Professional
  • Evidence of specialised continued professional training in clinical practice.


Experience

Essential

  • Demonstrating supportive and sensitive communication to patients, carers and staff, whilist demonstrating an understanding of the nature and effects of some information and barriers to effective communication whilst being considerate of client confidentiality.
  • Sound knowledge of clinical/risk assessment and understanding of Information Government principles
  • Sound knowledge of the national agenda for mental health.
  • Relevant experience of working with mental health needs


Desirable

  • Previous experience working as a registered mental health nurse with people with mental health problems.
  • Personal experience in the delivery of physical care to others


Skills & Competences

Essential

  • Highly motivated and able to engage with service users and carers to improve outcomes.
  • Sound clinical reasoning skills
  • Ability to work independently and collectively.
