An exciting opportunity has arisen for a Band 5 Neonatal Discharge Coordinator to join the dynamic and supportive team in our local Neonatal Unit at Buckinghamshire Healthcare Trust, based at Stoke Mandeville Hospital.
As a member of our team you will be professional, reliable, safe and compassionate to babies and their families. You will work collaboratively within our multidisciplinary team in an environment that is open, supportive and empowering.
The Neonatal Unit at Buckinghamshire Healthcare NHS trust is a Local Neonatal Unit comprising of 16 cots which include 2 intensive care, 3 high dependency and 11 special care cots.

The Neonatal Unit at Buckinghamshire Healthcare NHS trust is a Local Neonatal Unit comprising of 16 cots which include 2 intensive care, 3 high dependency and 11 special care cots.
Main duties of the job
- To be highly competent, knowledgeable practitioner ensuring provision of excellent evidence-based care
- Follow the discharge pathway within the neonatal service and provide specialist advice and guidance to parents and colleagues about the discharge process.
- Maintains close working relationships with nursing and medical staff and allied health professionals to facilitate discharge planning.
- Facilitate a seamless discharge of babies with complex needs from the neonatal unit (NNU) by encouraging multi-agency collaboration.
- Supervise and teach junior staff and pre and post registration student whilst maintaining a safe working environment.
- Work within the codes of practice, professional guidelines, and policies.
- Promote the continuous improvement of clinical standards in conjunction with the application of evidence-based practice.
- Be responsible for the information governance regarding discharges for NNU and the completeness of data.
- Works independently and as a member of the multi-disciplinary team to develop high standards of neonatal care.
- To comply with all Trust policies and procedures including health and safety, safeguarding and manual handling.
- To report incidents in line with Trust incident policy.
