The Health Inclusion Team (HIT) works across Lambeth, Southwark, and Lewisham to provide a multidisciplinary service for clients who are marginalized and have difficulty accessing primary and secondary care services (e.g. refugees and asylum seekers, homeless clients, and clients with addictions). The nurses have a broad range of advanced clinical skills and work autonomously in clinics in non-NHS sites, so have to be competent and safe in their clinical decision-making. We currently have a vacancy for a Band 6 Community Children's Nurse Specialist covering families and children in the Asylum Seekers Hotel service. The Hotel Team is a multidisciplinary team, in-reaching into the hotels and offering core health assessments, minor aliment treatments, responding to immediate needs, and linking the residents into primary and secondary care. The team has been established for one year and have already established themselves as an award-winning team. As a member of the team, you will be delivering a direct service in the hotels. The families/children can have complex health and social care issues, so you must have an understanding of their health needs and be confident when managing risk and safeguarding.

Main duties of the job

The Health Inclusion Team works across Lambeth, Southwark, and Lewisham to provide a high quality comprehensive specialist nurse service for clients who have difficulty accessing primary care services e.g. refugees and asylum seekers, homeless clients, and clients with addictions and high users of A& E. The Health Inclusion Team aims to assist these clients to access mainstream health services.

Key services provided are:

• Assistance with GP registration,
• Children's Health screening (i.e. TB, blood borne virus screening, sexual health screening, baseline routine blood tests)
• Comprehensive health assessment
• Minor illness / injury triage & management
• Wound care
• Vaccinations
• Chronic disease management
• Harm reduction work
• Case management of complex clients
• Referral to other services as appropriate

The post holder will be covering accommodation for asylum seekers who are housed in the hotels for asylum seekers and bridging hotels for resettlement families and individuals and refugee services.
