Our friendly and dynamic Older Person's Community Mental Health Team are looking for a talented and motivated Mental Health Nurse to join us in delivering high quality care to our service users and families.
We aspire to provide excellent services, with person-centred care and recovery-focused interventions for our service users. We have an established, experienced and supportive team who you would work with and who can provide the necessary framework to sustain and develop your skills and interests.
We are looking for either an experienced Band 5 or Band 6 Practitioner who is keen to work with Older Adults with mental health needs in the Community. They will be able to demonstrate an excellent knowledge of organic and functional mental illness as well as having some knowledge of physical health problems. They will also be expected to contribute to the wider working of the team, which would be participating in the duty system and supervising more junior staff.
Ability to drive and use of a car for work purpose is essential in this role.

Main duties of the job
Leading and contributing in the process of assessing, planning and coordinating service user care and undertaking interventions in line with agreed care plan.
Delivering higher level intensity community interventions as part of the integrated Shared Care model.
Formulating care plans in line with assessed need.
Working in a manner which is helpful to the service user and the referrer, based on principles of recovery, personalisation and choice.
Working with individuals in the least restrictive manner possible.
Deputising for the Team Leader as required.
You will have previous experience in the effective and positive management of people with mental health needs and will be able to evidence effective team working and leadership skills.
Supporting/supervising junior members of Staff and providing mentorship for Students.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
