Responsible for the delivery of advanced nursing skills within complex patient care pathways. This includes having continuous responsibility for the assessment of care needs and the development and implementation and evaluation of patient care establishing evidence-based practice.
To manage, develop and co-ordinate the service providing support and management of patients during assessment for Home Therapies Treatment including Peritoneal Dialysis and Home Hemodialysis within both the hospital and community setting.
Be responsible for enhancing the Home Therapies Programme. This will include providing:
A comprehensive assessment of patients who wish to undertake a Home Therapy Treatment
Education and support for all potential patients and careers
Provide Managerial/Clinical Support to the Home Therapies Unit
Provide specialist knowledge and skills regarding Peritoneal Dialysis and Home Hemodialysis, providing support to patients, their families, and staff. This will include responsibility for teaching, mentoring, and assessing students, other trained staff, trainees, and professional staff and promoting a learning culture.
This will be achieved whilst working in conjunction with the relevant Consultant Nephrologist.
Main duties of the job
To ensure an appropriate referral pathway to the Home Therapies Team is in place. To allow all renal patients to be assessed, establishing their suitability for Home Therapies, in line with national standards and National Standards Framework
To facilitate the smooth running of Home Therapies Clinics both medical and nurse led ensuring that tests and investigations are followed up appropriately.
Actively participate in local and national clinical audit programmes with support from Clinical Audit Officer
To undertake home visits as required/necessary to facilitate the education of patients/ family/carers regarding aspects of undertaking a Home Dialysis Therapy
To ensure local protocols/Standard Operating Procedures are updated and reviewed within agreed timescale.
To promote the health and wellbeing of patients undertaking a Home Dialysis Therapy by developing access to health promotion information in collaboration with other members of the multidisciplinary team
