We are currently looking for an experienced, knowledgeable and educated Advanced Clinical Practitioner to join our team on Adult Community Services - West Park Ward 2. You will be able to work autonomously making high level decisions to treat health needs as part of the wider MDT.
Ward 2 at West Park Hospital ismoving towards an ACP led structure, replacing the current medical model, to proactively manage those patients still in receipt of hospital based care, optimising their condition to support rapid onward transfer to the place they call home. There will be a requirement to facilitate ward rounds, huddles and undertake planned and unplanned clinical interventions to prevent a transfer back to the acute site. In addition, there is the opportunity to develop the escalation pathways that will facilitate a 2 hour response from the Community Urgent Care team, to prevent an ED attendance or hospital admission and contribute to ongoing evaluation of this new way of working.
The role will be integral to ensuring that West Park continues to be a vibrant community hospital that offers specialist rehab across the Black Country, and the successful candidate will need to work collaboratively with the ACS management team and West Park clinical teams to deliver this.

The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role and service. The post holder will support the Lead Advanced Clinical Practitioner in developing a new way of working that emphasises a more efficient and patient focused service and will ensure the safe referral and discharge of patients. This post is firmly grounded in direct compassionate care provision or clinical work with patients, families and populations.
