An exciting opportunity has arisen within Southern Health Foundation Trust to appoint an inspirational and enthusiastic Staff Nurse to join the out of hours nursing service. The applicant will be required to have a flexible approach as they may be asked to work a mix of Twilights and occasional nights. Working Twilight 5pm till 23:30 and night response 10pm till 04:30am. We have a 12 hour contract 2 nights/ twilights per week with scope for excess available within the service The out of hour's response teams cover all the GP practices across the Fareham, Gosport and East Hants localities. Applicants should be aware that the base for work can be in either GWMH or Rowans hospice and cross cover is sometimes required to ensure adequate service provision.
You will be working alongside an experienced auxiliary nurse within the surrounding localities responding to client needs directly. The work is varied and interesting including end of life care, falls prevention and catheter management. You will be responding directly to clients' needs and liaising with the Community care teams in the area. You will also be liaising with the out of hours GP services, SCAS and 111 in the area to promote hospital discharges and prevent hospital admissions as part of our urgent care service.
12 hours a week.
Access to a vehicle and possession of a full driving license is essential.

Main duties of the job
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
