Surrey Downs Health & Care
We are looking for an enthusiastic, experienced and highly motivated Care Home Liaison Nurse Band 6 to join the Care Home Support team working in SDHC. Your expert clinical skills, knowledge and professional growth are key to ensure high standards within community services are maintained.
Working across organisational boundaries and as an integral part of the Surrey Downs Health and Care System, the integrated health and care team will be responsible for providing rapid access, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced short term support as one stage in their on-going care pathway. The Home First Service operates as a 7 day service.

Main duties of the job
- Establish and foster improved working relationships with local partner organisations that have similar interests/functions in improving the quality of care homes and discharges from the acute hospital.
- Reduce the need for care homes to escalate care unnecessarily to A& E, nursing home setting, acute inpatient facility or consider discharging clients prior to exhausting a coordinated strategy to stabilise and improve the case concerns for both clients and care homes.
- Recognising the relationship with other services with the SDHC organisation and appropriately referring on to those services where assessed as appropriate (UCR,VW,CTT and QICH)
- Deliver well-being and recovery focussed interventions in the least restrictive care plan possible, in collaboration with the care home staff, clients families
- Support service users in their transition from home or other setting to a care home the post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective liaison nursing service to patients residing in a Care home. This includes those being discharged from acute, tertiary care or transferring from the community to a care home setting. Working in partnership with Care Home staff the post holder will support General Practitioners and other colleagues to support the transition either to a new care home or returning after and admission to the Acute .

- Post registration experience
- Good understanding of the changing health and care environment

- Evidence of a sound understanding in, quality assurance, service improvement methodologies, change management and risk management
