Herefordshire and Worcestershire Heath and Care Trust are looking to recruit a compassionate and dynamic Advanced Clinical Practitioner to join our team of ACPs working across 5 Community Hospitals. We are seeking an enthusiastic and motivated clinician to help develop this ACP led service delivering medical care for patients from sub-acute care to general rehabilitation, and including specialist stroke services, palliative care and fractured neck of femur rehabilitation. This diverse clinical case mix requires dedicated practitioners with comprehensive assessment and medical management skills to deliver safe effective care. We are building a team for the future and recognise the benefits of ACPs leading patient assessment, management and interventional therapies, supported by a team of GPs. A Band 8a position will be offered to candidates who have completed an MSc in Advanced Practice and can demonstrate experience working at an advanced level within a relevant clinical environment. Main duties of the job
The successful applicant must possess Advanced Health Assessment skill and Level 7 and be a V300 non-medical prescriber as a minimum. You will work autonomously to admit and assess individual patient needs, determine a differential diagnosis, request investigations, and initiate appropriate holistic, research-based treatments, within a patient centred clinical management plan, include independent prescribing of medications. You will be expected to exercise advanced clinical expertise, and high levels of decision making in clinical care, whilst working closely with Medical, Nursing and Therapy teams to ensure patients receive appropriate holistic care, and co-ordinate a safe discharge with minimal avoidable delays. Additional responsibilities will include monitoring and leading on improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, facilitation of learning through teaching and supporting professional colleagues, and demonstrate enhanced professional leadership skills.
This is a full-time position, however a minimum of 30 hours worked over 3 days would be considered.
