Southern Health Foundation Trust has an exciting opportunity for a full-time experienced Practice Nurse to join The Willow Group, a forward thinking and innovative GP group providing primary care services for 49,000 patients over 5 local sites in Gosport.

We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously.

You will ideally have a qualification in childhood immunisations, travel vaccinations, wound care and cytology.

Your role will be to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. The focus of the role is the provision of preventative health care to the practice population. As an autonomous practitioner you will be responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within the general practice team and the wider community teams to meet the needs of patients and support the delivery of policy and procedures.

Main duties of the job

As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team and the wider community teams to meet the needs of patients, support the delivery of policy and procedures and provide leadership where required.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
