An opportunity has arisen in the NHS@Home team, based at University Hospitals Bristol and Weston Hospitals. With growing evidence that supports the benefits of a 'home first' approach for our patients, there has never been a better time to be part of the expanding NHS@Home service.

We are looking for Band 5 nurses to join our small but friendly multidisciplinary team of nurses, pharmacists, AHP and Consultants. The NHS@Home service provides a 7-day service, currently 0800-1830 (no nights) but we can offer flexible working arrangements.

We have roles based at Weston General Hospital AND Bristol Royal Infirmary but provide care for patients living in Bristol, North Somerset and South Gloucestershire.

Main duties of the job

NHS@Home acts as an extension to our UHBW Hospitals in Bristol and Weston, enabling patients to go home to continue their hospital treatments in the comfort of their own homes. The B5 role in our team will allow you to continue to utilise and develop your acute clinical and leadership skills whilst applying them to the care of patients in their home setting.

As nurse in the NHS@Home team, you will be involved in community-based care, currently Outpatient Parenteral Antimicrobial Therapies, Frailty, Acute Respiratory Infection, Heart Failure and General Surgical/Medical, providing care for our patients in the place they call home.

You will work autonomously, delivering individualised treatments and care to our patients in their own homes enabling our patients to leave hospital sooner and prevent hospital admissions.

This role will enable you to continue to utilise and develop new clinical skills, knowledge, and expertise and we will support you with the training you will require (such as managing elastomeric devices and PICC removal).

Job responsibilities

For this post you must hold a full UK NMC registration and will be expected to hold a valid UK driving license and have a car to use daily with a valid insurance certificate covering business use as you will be required to use your own vehicle for daily visits. Your travel expenses will be reimbursed as per Trust guidelines, and we will provide residential parking permits.

Please note: Full time or part time working patterns considered, weekend work but no nights.

The Essential criteria are:

Current and full UK NMC registration

Current UK driving licence, valid insurance and have access to a car as daily travel is required across Bristol, Weston, and surrounding areas.

Experience of accessing vascular access devices

Excellent patient assessment and clinical skills

Experience of working within a multi-disciplinary team

The ability to work alone will be required during the normal course of this role.

Flexibility and appropriate experience are essential.

Desirable Criteria:

Previous experience of working in the community setting is desirable but not essential as we will provide training.

Experience with supporting patient education around self-administration skills

Interested in innovation, improving patient experience, and safety by exploring different models of care that support patients to be treated in their preferred place of care.

You will have excellent time management and be able to prioritise your own workload.

Person Specification

Knowledge and Experience

Essential

  • Holds a full UK driving licence and has access to an insured car daily to complete community visits
  • Evidence of continued professional development i.e., a professional portfolio and participation in appraisal and development review
  • Evidence of attendance at mandatory training appropriate to the role
  • Sound knowledge of health promotion issues to ensure junior staff, patients and carers are provided with education, support and advice as required
  • Evidence of experience and knowledge of the importance of resource management including protocols for staff utilisation, equipment, and products for clinical care


Desirable

  • To have achieved a recognised teaching & assessing qualification
  • Completion of training for additional clinical skills required in the service i.e. IV drug administration, cannulation, venepuncture and wound care (not exhaustive list)
  • Training for the use of pumps or machinery relevant to the service


Skills and Abilities

Essential

  • Good basic level of English language demonstrated through effective written and verbal communication skills
  • Ability to solve problems and use initiative
  • Ability to develop own clinical skills and a willingness to participate in continuing professional development
  • Competency in medication administration including IV drug administration or willingness to undergo training
  • Basic IT skills (e.g. Microsoft packages)
  • Ability to take the lead, initiate and ensure appropriate action in response to the deteriorating patient

Qualifications and Training

Essential

  • RGN.1st Level or relevant children's qualification
  • Current NMC Registration

  • Training Qualification or equivalent experience
  • Relevant post registration course
