We are looking for an enthusiastic and experienced Band 7 Physiotherapist to work in the Brent Rapid Response Team, willing to develop their skills and push themselves outside the normal boundaries of practice and with a genuine drive to ensure their patients get the best treatment possible. Brent Rapid Response is seven days a weekly service, 365 days a year, working from 8:00 until 20:30 or 10:00 until 22:30. You will be expected to work weekends and bank holidays. Service aims to bring the best hospital care to a patient's home and therefore avoid unnecessary hospital admissions for adults over the age of 18 years. You will work as part of a multidisciplinary team alongside therapists, doctors, dieticians, and pharmacists to provide integrated and personalised care to the patients.

The successful candidate will have relevant experience working as a band 6 in acute and community settings. As a member of the team, you will demonstrate clinical leadership within the clinical environment. You will be expected to work alongside the Team leader to maintain quality and safety standards.

Must hold UK HCPC registration and valid UK driving licence with a car.

Main duties of the job

  • To undertake comprehensive head-to-toe assessments without supervision, determine patient needs/risks, and plan, implement, and evaluate their care with support from service consultants.
  • In the absence of senior colleagues, take charge of the unit and manage the team day to day, including taking handovers and helping to make treatment plans at the virtual ward round with the consultant.
  • To work in partnership with multi-disciplinary members of the team, other professionals, and agencies, to meet the health and rehabilitation needs of patients within the Brent Rapid Response service.
  • To maintain standards of clinical care using an evidence-based approach within the service, incorporating principles and values of clinical governance.
  • Ensure effective communication is always maintained whilst providing support, supervision, and clinical expertise to all members of the team.
  • Develop your skills into a more well-rounded holistic approach to patient care. Take on extended roles such as venepuncture, drug administration, advance assessment of the acutely ill patient and acute injury assessment.

Education / Qulaification

Essential

  • Diploma/degree in Physiotherapy
  • Current HPC registration
  • Experience working at a senior level in a wide range of clinical areas including, neurology, general rehabilitation, falls, frailty, elderly medicine


Desirable

  • Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation or older people
  • Post graduate degree in relevant area
  • Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment.


Experience

Essential

  • Experience working at a senior level in a wide range of clinical areas including, neurology, general rehabilitation, falls, frailty, elderly medicine.
  • Extensive experience in Primary/Secondary Care and/ or Accident & Emergency.
  • Experience in assessing and providing quality care to people with Long Term Conditions
  • Proven ability to prioritise and manage own workload with minimal supervision.


Desirable

  • Experience of wide range of specialist services eg. falls services, wheelchair services, and learning disabilities.
  • Experience working as a Band 6 Physiotherapist
  • Experience of working in the NHS


Skills and Key attributes

Essential

  • Skills in the assessment and treatment of adults with complex therapy needs
  • Good communication and report writing skills, in both verbal and written format
  • Ability to apply core physiotherapy skills across a range of clinical conditions within a client centered, goal setting
  • Knowledge of outcome measures and evidence based practice relevant to Physiotherapy practice in community
  • IT skills


Desirable

  • Supervision of assistant / junior staff
  • Knowledge and experience in research and audit
  • Car driver
  • Able to work flexible shifts and long days.
  • oAble to work across different services when required
