Do you want to be part of a new model of community mental health care working across an Integrated Care System?
Join team BSMHFT where our vision is simple - to improve mental health well-being, and our values of Compassion, Inclusion and Commitment are promoted in everything we do.
If you are dedicated to providing partnership care and being part of an innovative development of Neighbourhood MDTs, in a city enriched by cultural diversity, join our teams at Lyndon Clinic CMHT, Solihull.
In return for your commitment, expertise, creativity, and ambition we will invest in your continuing professional development, encourage you to develop special interests, undertake research activities, forge links with academic centres or progress managerial aspirations to help you reach your full potential.
Main duties of the job
The Lyndon CMHT is one of two CMHTs in the Solihull Community Hub. The CMHT looks after adults over the age of 25 and has links to other teams within the Community Hub which includes the Older Adult CMHT and Assertive Outreach Team. There is also a dedicated Home Treatment Team that is geographically linked to the CMHT.
It is anticipated that referrals and caseload numbers are likely to reduce as we progress with our Community Mental Health Transformation as part of the Long-Term Plan. There will be less focus on static caseloads and more focus on partnership working in local neighbourhoods aligned to Primary Care Networks.
