The Specialist Children's Community Nurse- Complex Clinical Needs will work within the Children's Community Nursing Team in the community settings across Bedfordshire and Luton.
You will lead, develop and implement pathways for the nursing care of babies, children and young people with complex clinical needs across the Children's Community and Specialist Nursing service.
Supporting teams embed the relevant pathways into their clinical practice, and to provide education and training using your expertise.
It will essential to develop positive working relationships within an integrated community network to influence patient and professional experiences to ensure best outcomes.

Main duties of the job
Be an expert resource for Children's Complex Clinical/ skills and knowledge, providing education and training to the CCNSN Team and the wider multi-disciplinary team.
You may be required to participate in the on-call rota for the service.
Be the lead Clinician within the CCNSN Team for the specialised area of children's complex clinical care including networking with specialist nurses to develop practice and sharing knowledge and skills with colleagues.
Work in conjunction with the named professionals for children/young people with complex clinical, acting as a resource to improve healthcare outcomes
Lead, develop and establish integrated clinical care pathways across CCNSN services to ensure a consistent approach to case management/care coordination.
Work with the Children's Services Team Managers ensuring that the resources and skills of the team are used effectively and appropriately to meet the needs of the children/young people and their families, helping to identify any gaps in service provision.
Liaise and work collaboratively with professionals in primary, secondary and tertiary healthcare settings to ensure timely discharge planning.
Co-ordinate/facilitate the implementation of complex symptom control regimes plan and lead specialist complex community nursing interventions utilising a range of clinical skills.
Support the implementation of transition pathways from Children to Adult Services for children with complex needs, ensuring a seamless pathway

You will be expected to practice as a prescriber within an agreed scope of practice as part of this role.
Treat symptoms through pharmacological and non-pharmacological strategies. Undertake diagnostic testing within competence, e.g. Phlebotomy.
Empower health and social care professionals to deliver quality evidenced based care service by sharing knowledge and expertise.
To work therapeutically with children, young people, their families and other professionals
To develop and sustain effective working partnerships with local Clinicians, other specialist care providers, managers and team colleagues.
Understand and ensure good practice in managing patient information between the different care settings and teams.
Understanding of IT with an ability to use a range of IT systems that aim to improve communication within the services, including electronic patient records.
Budgetary Responsibility
To work with the Team Manager to ensure service remains within budget.
People Management
Responsible for day-to-day supervision of all staff as a senior nurse across the services. Act as a supervisor for staff managing children and young people with complex clinical care needs
Responsible for day-to-day management of a group of staff
Responsible for the development of staff across the countywide CCNSN service caring for children with complex clinical care.
Maintaining own professional development and requirement to take part in appraisal and KSF process.
Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
The post holder must adhere to the Trust risk assessment and risk management processes.
The post holder must adhere to infection control policies and procedures.
It is a condition of your employment that you are currently registered with your professional body, and it is your responsibility to maintain your professional registration.
Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
The post holder must participate in clinical and safeguarding audits as required.
Provide patients and their families /carers with information on standards they should expect from the team.
Participate in Clinical & Safeguarding supervision on a regular basis.

- Working in the Complex clinical care field with babies, children, young people, with sound knowledge of symptom management and assessment of their health needs.
- Evidence of teaching/training experience
- Experience of Project management principles including Audit /evaluation of service/project. implementing / reviewing clinical care delivery changes in practice.
- Ability to evidence knowledge on the wider Complex clinical care agenda.
Desirable
- Experience of caring for children and young people with long term conditions
- Experience of working with families and carers to promote health and wellbeing of their CYP.
Skills
Essential
- Wide range of extended clinical skills relevant to complex care e.g. syringe drivers, tracheostomy, CVAD's.
- Proven physical assessment skills.
- Ability to demonstrate good IT skills.
- Excellent communication and interpersonal skills - written and verbal with evidence of basic counselling skills
- Working as part of a multidisciplinary team, developing, and sustaining supportive relationships.

Qualifications and training
Essential
- RSCN/ Registered Nurse (RN) Child Branch - with evidence of post qualifying education (to a PG dip level (Level 7) or evidence of working at that level.
- RN Adults, RN Learning disabilities or Registered Midwife with SCPHN, SPQ or ACP qualification, WITH extensive evidence of Managing children with Complex clinical / Long term conditions with post graduate training in the community or willingness to complete within two years.
