The North Ceredigion Cluster is one of the seven Primary Care Clusters in Hywel Dda University Health Board and is located in Ceredigion, which is mainly a mainly rural and coastal area, with Aberystwyth, a University town situated at the heart of the cluster. Geographically it expands from Aberaeron to Borth and across to Tregaron, and includes 7 GP practices and a General Hospital.

The North Ceredigion Primary Care Cluster is seeking to appoint an enthusiastic and innovative experienced Physiotherapist with excellent clinical and communication skills to join our new multi-disciplinary Chronic Pain team.This is an ideal opportunity to be part of a new and actively developing service where emphasis is on delivering the best possible care for the people of Ceredigion, whilst promoting innovation, research and values based health care in all that we do.

We are fortunate to have very active clinical and professional networks on an All Wales basis with direct links into the Welsh Assembly Government, local Universities and service delivery groups.

Main duties of the job

As a Specialist Physiotherapist within this team, you will play a leading role in reviewing, auditing, developing and delivering patient care whilst being supported by other Senior Practitioners in the Chronic Pain service within Hywel Dda University Health Board and wider physiotherapy services and networks.

You will be actively interested and contribute to evidence based practice and service development.

You will be involved in providing training to other services and team members and have access to training and actively engage in clinical supervision.

You will also provide supervision and mentoring to other team members as required.

Job description Job responsibilities

The post holder will provide clinical leadership and specialist physiotherapy intervention and expertise in the delivery of multi-disciplinary care within a biopsychosocial framework for clients living with persistent pain referred within the Primary Care Cluster.

The post holder will lead on the delivery of the Chronic pain and physiotherapy plans for their specialty and overall this will support the Chronic Pain Service and Physiotherapy service plan for Hywel Dda.

Develop and maintain clinical standards across the Health Board

Develop a competency based framework for physiotherapy practitioners and other members of the multi-disciplinary team as is appropriate and within competency. This will include providing skills based training within this specialty area.

Undertake critical analysis, appraisal and synthesis of current research outcomes and/or relevant literature in order to maintain expert levels of knowledge and practice within specialty area.

The post holder will autonomously manage a caseload of clients with highly specialised complex needs, using evidence based/client centered principles to assess, plan, implement and evaluate interventions in primary, community and secondary care settings.

They will also play a key clinical leadership role in supporting the Chronic Pain and Physiotherapy services, modernizing services by contributing, planning, developing and evaluating the service within their area of specialism This will include leading in audit and evaluation.

The post holder will lead and participate in CPD and other development issues with staff, including being responsible for the supervision, training and educational packages for staff within their specialist area across the Health Board.

The post holder will undertake a team leader role by planning, coordinating delivery and evaluating the physiotherapy service in their specialty in collaboration with physiotherapy service leader. This will include performance management.

The post holder will be required to participate in the provision of a flexible 7 day service, based on service need. This will include weekend working as part of a rota.

The post holder will be required to participate in on call services on a rota basis in the context of service need.
