Community Mental Health Practitioner Dual Diagnosis

As a Dual Diagnosis Mental Health Practitioner, you will work within a proactive and positive multidisciplinary team, focusing upon individuals (including young people) with severe and enduring mental health problems who have co-occurring Substance Misuse issues.

You will have responsibility for the care co-ordination and management of a defined caseload of clients with complex and challenging needs, providing liaison between statutory and other services particularly with colleagues providing substance misuse, housing and homelessness support and working closely with social workers and disability teams (social services).

Main duties of the job

You will be an integral member of the Team, providing training and specialist advice as and when needed to other staff and undertaking interventions as defined within Parts 2 and 3 of the Mental Health (Wales) Measure 2010. Assisting the CMHT Team Manager with the management and organisation, including advice and support, of their clinical area associated with dual diagnosis will be an essential element of the role.

Qualifications and/or Knowledge


  • Registered Mental Health/ (Dual Diagnosis) Nurse and Registered with the NMC
  • Registered Occupational Therapist Registered Social Worker
  • Educated to diploma level
  • Evidence of continuing professional development
  • Advanced knowledge of current Mental Health Legislation and practice
  • Knowledge of Health & safety Legislation
  • Extensive knowledge of mental health and substance misuse issues
  • Knowledge of research based and evidence-based interventions
  • Advanced knowledge and application of the care and treatment plans
  • Knowledge of the Mental Health Act 1983 for those parts of the Act that directly affect the provision of Community Mental Health Services


  • Teaching and Assessing Qualification
  • Evidence of personal development to degree level or equivalent to degree level
  • Specialist practitioner award
  • Psycho Social Interventions qualifications
  • Cognitive Behavioural Therapy qualifications

  • Experience of community mental health working in nursing, social work, OT or other allied profession
  • Evidence of supervisory experience
  • Sound understanding of community and multidisciplinary, multiagency work
  • Experience of working within a community setting and with complex clients, older adults, adults, (including young people) who have a dual diagnosis of substance misuse and mental health issues
