South Falls Team

Community Occupational Therapist

Salary: Band 6 x 2 posts

Hours per week: 1 x 37.5hr and 1 x 27.5 ( part time hours will be considered and can be discussed)

Job Type: permanent

An exciting opportunity has arisen for Band 6 Occupational Therapists in the South Falls Team. As an autonomous Occupational Therapist you will work as part of the multidisciplinary falls team caring for adults with an identified risk of falling or with a history of falls. This post is based at Salters Meadow Health Centre in Burntwood and involves community working across South Staffordshire with adults in their own homes.

You will need to be a positive, enthusiastic and innovative individual who possesses excellent communication, organisational and IT skills. You will have knowledge and experience of working in a busy integrated team, of completing functional assessments and goal setting to promote and maximise people’s independence.

The team is dynamic, dedicated to deliver excellent customer service and is driven by outcomes and efficient ways of working.

You will have:

• A diploma or degree in Occupational Therapy and be registered with the HCPC

• An CPD portfolio and be committed to CPD

• A desire to work in the specialist area of falls prevention

• Excellent IT skills

• Confidence within group work settings and delivering presentations

• A full UK driving licence and have access to a car during working hours

• Experience in the NHS.

• A desire to demonstrate that interventions have made measurable outcomes to service users and their carers.
