JOB PROFILEThe post holder will care for patients in the community and work within the Ambulatory Outreach Team (AOT). Assist in the development, implementation, delivery and evaluation of the AOT [Acute Hospital at Home (AHaH) & Complex Outpatient Parenteral AntimicrobialTherapy (COPAT)] function and contribute in further development and streamlining of the AOT. As a senior nurse, the post holder is expected to work and provide specialist input, and flexibly rotate within both services.

The post holder is expected to work autonomously and support other team members. Care for patients in their home environment, support and improve the patient discharge process for patients who can safely be managed at home with a defined care plan. The post holderneeds to be able to perform holistic clinical assessment for Admission Avoidance patients (training will be provided) at the patient's home.

As such our Ambulatory Outreach Team and Hospital at Home service is expanding as we take on more work including covid work streams. Existing band 6's ready for a new challenge and the opportunity of learning new clinical skills. Due to the nature of our work a min of 2 years practice in an acute area of work is a must.

The Ambulatory Outreach Team (AOT) comprises of numerous roles which include Hospital at Home Central, Hospital at Home Horton, OPAT (Outpatient Parenteral antimicrobial Therapy) and Covid Care at Home. We also work closely with the Ambulatory Assessment Unit (AAU) at the JR and Rowan Ambulatory Unit (RAU) at the Horton. Therefore, there is a variety of work including hospital-based work, community work and the opportunity to work from home with covid care at Home service.

Job description

Job responsibilities

We are responsible for providing an alternative to acute hospital admission, treating and monitoring patients so that they can remain at home or in their care home during acute illness. This includes supporting care homes and patients with covid-19 with visits and following them after discharge through our covidcare@home virtual ward. We also facilitate early supported discharge from hospital, providing care that would previously been provided in an acute setting.

We can provide: Point of care blood testing, point of care ultrasound (POCUS), and we can guide clinical management decisions, non-medical prescribing, intravenous therapy, drain management, nebulisation, clinical reviews, insulin administration, support with care, management of heart failure/COPD. The team consists of visiting Ambulatory Consultant, Specialist Nurse Practitioners and Paramedics with a dedicated team Pharmacist. Our teams aim is to provide treatment or care in the patient's home that is equivalent to inpatient care. We work collaboratively across the whole of Oxfordshire with all hospital at home services and the Urgent Community Response Service (2-hour response). We are a fast-developing service, now Neutralising Monoclonal Antibodies (nMAb) treatment is starting within our service for covid infection patients who are immunocompromised and may not have mounted an adequate response to covid vaccination.

The successful candidate will need enhanced clinical decision-making skills, as we take our specialist nursing skills into individuals homes. They would be responsible for assessing patients, planning their care, administering treatment, and discharging or referring patients to other appropriate healthcare services. As part of Covid Care at Home we will use these skills to triage patients virtually and discharge or escalate patients care needs as appropriate.

We are a supportive service and training will be provided. We have regular teaching sessions and days to enhance staffs development and there is opportunity for development through advanced courses at Brookes such as the Advanced History Taking course.

  • Evidence of ongoing professional development through maintenance of an NMC revalidation or equivalent
