We are looking for a Band 6 Care Coordinator to join the integrated outreach team in Bradford.

You will either be an a qualified Occupational Therapist, Registered Social Worker or Registered Mental Health Nurse.

The service aims to: Improve and maintain our service user's engagement with services.

Our people will have a primary problem of psychosis and many will experience problematic co-occurring substance use.

We aim to provide trauma informed care and in so doing, promote and support the recovery needs of our service users.

The team strives to adopt an assertive approach to engagement and case management. Failure to engage will not always lead to case closure. Understanding and respecting individual values, beliefs, choices, and priorities underpins our approach to engagement. Relative to the CMHT our smaller caseloads can provide capacity for more frequent contact than would be possible in mainstream services offering more time to be creative and resourceful and more time for working with families.

Successful applicants will be able to demonstrate a high level of commitment, motivation, and knowledge of working with people with severe and enduring mental health. You will need to demonstrate clinically effective care management skills and interventions and will have a good awareness of current health and social care issues which affect, or influence, our community.

Person Specification



  • Professional qualification in mental health nursing, social work, occupational therapy
  • Degree level education or equivalent experience
  • Registration with Professional Body (e.g. NMC, SWE, BAOT



  • Worked as keyworker with mental health client group
  • Minimum of 6 months post basic experience in Mental Health


  • Experience of being clinical adviser to junior staff or students
  • Experience in relevant area of Mental Health
  • Involvement in innovative practice
