We are looking for a Band 6 Care Coordinator to join the integrated outreach team in Bradford.

You will either be an a qualified Occupational Therapist, Registered Social Worker or Registered Mental Health Nurse.

The service aims to: Improve and maintain our service user's engagement with services.

Our people will have a primary problem of psychosis and many will experience problematic co-occurring substance use.

We aim to provide trauma informed care and in so doing, promote and support the recovery needs of our service users.

The team strives to adopt an assertive approach to engagement and case management. Failure to engage will not always lead to case closure.
