Work alongside the multidisciplinary team working within the UCR team. Responsible for the day-to-day management and coordination of team workloads within the UCR. Responsible for developing the UCR medicines management training programmes for all staff in the technical aspects of medicines management role in line with national standards. Work with the trusts Lead Technician for Medicines Management and assist with the development and provision of the technical aspects of the medicines management service across primary and secondary care. Review, write, develop, and implement policies and SOPs relating to the technical aspects of the medicines management role in the trust under the direction of the Lead Technician for Medicines Management. Be guided by inpatient departmental procedures, national guidance, good clinical practice, and hospital policies. To provide a technical role alongside Nurse Consultants and other healthcare professionals in a UCR environment to support patients in the management of their medication including:Medicines reconciliationAssessing patient's own drugs for suitabilityRequest supply of medication for individual patientsChecking medication on dischargeCounselling and educating patients on their medicationSupport the referral of patients to the Discharge Medication Scheme

Main duties of the job

To undertake accurate medication reconciliation in the Intermediate Care and Urgent Community Nursing Service setting. Responsible as an individual practitioner for providing a technical medicines management service to both the acute and community setting. Organise timely supplies of new, additional or absent medicines. Support timely access to medicines by establishing, ordering, reviewing, and reconciling medicines prescribed by UCR clinicians to patients from medicine grab bags and ward stock. To advise the patient on medication management. To promote principles of self-care for medicines administration. To provide specialist advice to patients regarding medicines management issues, contributing to patient care, education, and safety. To communicate specialist information regarding medicines to patients, carers, members of the multidisciplinary team, and other healthcare professionals. To work in collaboration with acute trust colleagues. Support the timely transfer of information to nominated community pharmacies for at risk patients via the Discharge Medication Service as per procedures. Provide advice on the availability of medicines to healthcare professionals working within the UCR team.Prioritise own workload to ensure the smooth running of the pharmacy/medicines management service and ensure turnaround times are met.

Person Specification



  • Experience working in a senior technician role
  • Experience dealing with the public
  • Experience of hospital pharmacy practice as a Medicines


  • Evidence of implementing changes



  • To be registered with the GPhC



  • BTEC and/or NVQ 3 in Pharmaceutical sciences
  • Higher level Clinical Pharmacy qualification e.g. NVQ level 4 or equivalent
  • Nationally recognised accuracy checking technician qualification


  • Management qualification or evidence of team management
  • Qualified NVQ assessor (A1, AQA)Level 3
  • Professional teaching qualification



  • Knowledge/competence using IT systems (MS Office)
  • To undertake Continuing Professional Development
  • Specialist knowledge acquired through training and experience
