Clinical Nurse Specialist Diabetes
We are seeking an enthusiastic and motivated nurse with an interest in Diabetes to join our established multidisciplinary diabetes team to help deliver high quality diabetes care to patients and healthcare professionals within the Trust.
You should be an excellent communicator and educator, have good organisational skills, work well as part of a team and want the challenge of gaining new skills and knowledge. You will be joining friendly, dynamic colleagues committed to providing high standards of care to patients with diabetes.
Applicants are expected to be registered on Part 1 of the NMC register. Internal and external diabetes training will be provided to ensure appropriate competencies and accreditation for diabetes education and DVW service delivery. Prior experience, accredited CPD in diabetes and evidence of specialist knowledge and independent diabetes working will be viewed favourably. Support will be provided in-post for continuing professional development, including but not limited to insulin pumps, independent non-medial prescribing and peer reviewed diabetes education delivery.
This post would suit any experienced nurse with experience in diabetes care and who wants to specialise in the field of diabetes and is looking for a new and exciting challenge with career progression opportunities.
Main duties of the job
The post-holder will provide evidence-based diabetes care and advice to patients with diabetes with responsibility for a patient caseload. In conjunction with Diabetes CNS colleagues, the post holder will oversee the newly initiated Diabetes Virtual Ward (DVW) including identifying and onboarding suitable patients to the DVW. The post-holder will liaise with colleagues across the primary-secondary care interface to support safe and efficient diabetes management and transfer of care. The post-holder will disseminate diabetes education to patients and healthcare professionals across the Trust as part of regular staff training programme. The post holder may be asked to work flexibly within a 6-day working week in conjunction with other Diabetes CNS colleagues.
