The Intensive Home Treatment Team at Airedale Centre for Mental Health are looking for a qualified Community Mental Health Nurse (Band 6) to join their busy team.

You will be responsible for the assessment, planning, implementation and evaluation of care to adult service users of all ages. This role includes, working with older adult mental health conditions in service users' home environment. You will also contribute to the supervision and development of junior staff and regularly be Shift Co-Ordinator for the team. This includes, taking and making referrals to and from other health professionals and acute care services.

You will have at least 2 years' experience as a registered mental health nurse. Ideally, working in an acute mental health setting. If interviewed, you should be able to demonstrate very good knowledge of relevant mental health practices specific to intensive home treatment team and all age adult mental health conditions. You should also be able to discuss guidance from NHS England / Department of Health forward planning for all age adult mental health service users.

Main duties of the job

  • Job Purpose:
    • To provide care and interventions, risk and safety management and safeguarding of all age adult patients involved with the Intensive Home Treatment Team (IHTT).
    • To engage in regular supervision with the Team Manager/Advanced Nurse Practitioner. Discuss, reflect and find solutions to challenges experienced in the role.
    • To deliver a high standard and quality service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
    • To work in support of other allied health care professionals and services in the assessment, planning, and evaluation of care and treatment.
    • To communicate effectively with other services of the Care Trust, other health care professionals and with the patient family, carers/ visitors to ensure holistic care is delivered.
    • To supervise and delegate work to Band 5 Nurses, Senior Healthcare Support Workers and Healthcare Support Workers.
    • To occasionally be Shift Co-ordinator in the team. Including, triage of referrals, inpatient services link worker and covering Bed Manager during normal office hours.

  • Main duties:
    • Provide evidence based clinical interventions, making autonomous clinical decisions about own professional practice.
    • Responsibility for the development, planning and implementation of care and treatment interventions for individual patients within the Acute Community Services, including, older adult service users.
    • Provide assessment, planning and implementation of individual packages of care and treatment.
    • Ensure that individual episodes of care are delivered in a timely, effective and integrated manner.
    • Promote a recovery model that empowers patients, carers and relatives to be at the forefront of decision making and ownership of their packages of care and treatment.
    • Undertake clinical risk assessments, based on latest empirical evidence and compliant with local policies and procedures.
    • Act as the patients and relatives advocate.
    • Enable patients and carers to manage disability, loss and change.
    • Champion patients rights, including: dignity, equality, diversity, choice and respect.
    • When undertaking all of the above ensure good documentation is provided and entered within the appropriate systems of record sharing.
    2. Working as part of a Team
    • Promote a culture that incorporates Trust values, is recovery focused and there to assist patients, carers, families, referrers and colleagues.
    • Provide timely, accurate and appropriate organisation information to the colleagues and the Team Manager.

  • Staff Supervision and Support
    • Monitor practice and development of Band 5 staff and Healthcare Support Workers
    • As required by the team manager, conduct appraisal and performance management of Band 5 staff and Healthcare Support Workers
    • Assist in induction of staff.
    • Provide clinical leadership for Band 5 staff and Healthcare Support Workers
  • Financial Responsibility

  • The post holder does not have financial responsibility for budgetary management. However it is the individual post holders responsibility to manage organisational resources efficiently and to suggest efficiency savings to line management as they arise. The post holder has the responsibility to maintain the safekeeping of the organisations property and safeguard the property of patients and visitors.
