Do you enjoy working within a fast paced environment, dealing with complex issues and ensuring those under your care receive optimum treatment? Burbage Ward are looking for those with a healthcare qualification (Occupational Therapist, Nurse or Social Worker) to take up the position of Discharge Coordinator to ensure that effective flow in-and-out of hospital occurs.

The successful candidate will be joining a diverse and proactive team to lead upon hospital discharge alongside the development of new and efficient ways of working that will lead individuals on their road to recovery. You will also be working not just within NHS services but will develop a wider relationship with other organisations, such as local authorities and third sector.

Burbage Ward is a busy Female acute admissions ward for those experiencing a mental health crisis, with discharge from hospital being discussed and planned at the point of admission. You will receive regular supervision and additional training to develop both yourself and the role, as well as being an active member of the management team on the ward.

Main duties of the job

This role includes the need to create strong working relationships with service providers internally and externally to the Trust with a clear emphasis on partnership working between community mental health services within SHSC. You will act as a positive role model in the ward team, ensuring high standards of practice and professional behaviour alongside supporting effective change procedures to support current guidance around care.

The post holder will work as part of the leadership and management team actively and positively contribute to the clinical governance and development of the ward processes and policies.

You will need to be a registered mental health professional with relevant registration (Nurse, Occupational Therapist, Social Worker)and post registration experience. The post holder will have a commitment to enhancing their own knowledge and skills whilst engaging the team in supervision for there development.

Job description

Job responsibilities

  • Responsible for the coordination and monitoring of care plans, ensuring all in-patients have effective plans at the point of hospital admission and are discharged in a timely manner with a smooth transition to other services
  • Responsible for maintaining a collaborative working interface with Home Treatment Team to identify patients suitable for discharge, and to work together to promote a smooth discharge pathway from inpatient services.
  • Work with external services in a timely manner so to identify practical help that can be provided to clients in achieving a prompt discharge.
  • Works as an effective member of the multi-disciplinary team, maintaining and actively role modelling high standards of care.
  • To develop effective changes in practice so to support in-patients receiving a high standard of care, meeting individual needs.


Person Specification

Qualifications

Essential

  • Registered Mental Health qualification (Nurse, Occupational Therapist, Social Worker) with relevant professional registration.
  • Commitment to enhancing own knowledge and skills.


Experience

Essential

  • Able to demonstrate an effective contribution to MDT working.
  • Demonstrate an awareness of current developments in clinical practice.
  • Demonstrate experience of supervising qualified staff.
  • Demonstrate evidence of the application of post basic education into clinical practise.
  • Demonstrate evidence of leadership on a service development issue.
  • Demonstrate risk assessment and risk management skills with service users and the environment.


Knowledge & Skills

Essential

  • Demonstrate a thorough knowledge of the Mental Health Act, 1983.
  • Demonstrate a thorough working knowledge of CPA and the Acute Care Pathway.
  • Demonstrate an understanding of the processes of change.
  • Demonstrate sound leadership skills, possessing the ability to direct and motivate staff.
  • Demonstrate excellent communication skills, both oral and written
  • Demonstrate evidence of good organisational skills.
  • Computer skills


Desirable

  • Skills in DBT, CBT or PSI
