An opportunity for a Registered Nurse with an interest in Cardiology to gain experience and skills within the community setting, working directly with the Specialist Cardiac Nursing Team. No previous cardiac experience necessary, just enthusiasm to learn and be part of a team.

The role is to support the CCCS team in providing clinical, team-orientated, specialist rehabilitation for people in Southampton and parts of Hampshire with Cardiac Disease, Heart Failure and associated conditions.

The service supports a cardiac rehabilitation programme for Southampton City and Hampshire patients with qualifying conditions (stent, myocardial infarction, surgery, valve replacement/repair and transplant). These sessions take place across Hampshire in four sites: Totton, New Forest, Shirley, and Hedge End.

Main duties of the job

To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment.

The focus of the Combined Community Cardiac Services (CCCS) is to ensure that patients are identified, treated and supported in the least invasive setting possible. Reduction of hospital admissions and reduced length of stay are key targets for this team.

The service supports a cardiac rehab programme for Southampton City and Hampshire patients with qualifying conditions (stent, myocardial infarction, surgery, valve replacement/repair and transplant).

The team has a GPSI led diagnostics service for patients registered with a Southampton City GP. The service offers Echocardiograms and 24-hour ECG monitors. There is a Cardiac Specialist Nurse Review Clinic for patients referred from UHS or who are registered with a Southampton City GP. This service offers advice and information on medications, post-surgery recovery and lifestyle guidance. Patients are also assessed for suitability of attending the cardiac rehabilitation programme.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Qualifications

Essential

  • BSc Nursing or RN currently registered with NMC.


Desirable

  • Practice assessor/supervisor.
  • Teaching Qualification, e.g. ENB 998 or equivalent.


Experience

Essential

  • Experienced in developing care plans
  • Experienced in developing and delivering care and clinical interventions to patients
  • Experience of MDT working
  • Experience of teaching, training and / or supervision
  • Evidence of recent CPD
  • Evidence of developing policy, guidelines contributing to management of resources
  • Clinically competent at Band 5 level
  • Excellent communication skills, written and verbal
  • Ability to prioritise workload and work autonomously, instruct and direct others work is managed, not supervised

  • Ability to travel for meetings across a range of sites in Southampton/Portsmouth/Hampshire as required for role.
  • For posts which require a professional registration, post holders hold a valid up to date registration with their professional body.
  • For posts where post holders are required to drive as part of their role they hold a valid full driving licence which enables them to drive in the UK.
