Bedfordshire Community Therapy Service (BCHS) is looking for a full time Occupational Therapist to support individuals within the Discharge to Assess Beds (D2A), Inpatient Rehabilitation Beds( IPR) and within their own place of residency within the community (home).
Main duties of the job
- Responsible for carrying out assessments, interventions and rehabilitation programs to promote an individual's safety and independence with support from senior staff.
- Develop confidence to identify concerns or risks that would impede an individual's safe discharge to their place of residency after a hospital admission or a short stay within a D2A or IPR Bed. Using problem solving skills, clinical judgement and collaborative working with system partners to resolve the concerns for the benefit of the client.
- Provide advice, equipment and evaluations based on a client's individual needs.
- To be responsible for a caseload which is safely prioritised with clear review plans in place.
- Experience dependent - To work as an autonomous professional according to the Royal College of Occupational Therapy (RCOT) and the Health and Care Professions Council's (HPCP) regulations.
- To work efficiently as part of a multi-disciplinary team and communicate confidently, asking for support where required.

- Grad. Diploma and / or degree in Occupational Therapy
