We have an exciting opportunity for a dynamic and enthusiastic Heart Failure Specialist Nurse to join our multidisciplinary heart failure team at Imperial.

We are an established, integrated heart failure service covering 3 acute hospital sites, one of which has a specialist Heart Assessment Centre. As an integrated service we run community heart failure clinics for our local population as well as managing patients in their own homes. We cover a diverse population of patients admitted to our hospitals and living in London. We provide specialist support and education for our colleagues managing patients within both primary and secondary care. We are committed to improving our heart failure service. We run a remote monitoring service for our heart failure patients and support both a Heart Failure virtual ward and day case diuresis/intravenous iron injections for our heart failure patients.

The service is fortunate to be led by both a committed Heart Failure Consultant Cardiologist and a Heart Failure Nurse Consultant and as well as a Heart Failure Lead Specialist Nurse. We have a specialist Heart Failure pharmacist and psychologist within the team.

We look forward to recruiting an individual who provides a high standard of evidence based care with experience of managing heart failure patients and who is committed to continually improving the service provided for heart failure patients. The individual will work collaboratively with the MDT to ensure that patients receive seamless care in hospital, at home and in the outpatient setting. Ideally we are seeking to recruit a nurse with experience in working autonomously with HF patients however If you are interested in starting a career in HF we are able to offer a band 6 training position with the aim of successfully achieving competence to move to band 7 in one year.All applicants must reflect a good understanding of the specialty and role as well as a strong commitment towards improving care within heart failure.

Main duties of the job

  • Follow NICE guidelines for the management of acute and chronic heart failure and support the service in achieving National targets.
  • Will be an experienced practitioner confident in managing a caseload of patients in a variety of settings.
  • As part of the multidisciplinary heart failure team will help to develop, implement and evaluate a seamless integrated specialist service, ensuring that patients receive the highest standard of evidence based care.
  • Demonstrate a high level of expertise within the service, providing specialist advice, education and support to patients and other health care professionals.
  • Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets.
  • Will be responsible for contributing to the ongoing development of clinical practice, teaching, research, clinical audits and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.

Person Specification

EDUCATION

Essential

  • Registered Nurse.
  • Post registration relevant qualification
  • Teaching and Assessing qualification eg 998 or equivalent teaching experience.
  • Evidence of further training to enhance scope of practice


Desirable

  • Heart Failure Course L7
  • Non-Medical prescriber or commitment to undertake
  • Advanced communication skills training


SKILLS/ ABILITIES

Essential

  • Able to demonstrate highly developed specialist knowledge within heart failure.
  • Knowledge of heart failure management both within primary and secondary care
  • Able to prioritise and meet deadlines
  • Knowledge of relevant national heart failure guidelines.
  • Has a wealth of insight into all aspects of heart failure
  • Excellent communication skills


Desirable

  • Demonstrate commitment through involvement in Trust wide initiatives/ committees.
  • Knowledge of research and audit including implementation of findings and review of outcomes


EXPERIENCE

Essential

  • Worked within cardiac/cardiothoracic nursing to gain a high level of cardiac experience.
  • Confident of the role within the MDT
  • Experience of teaching
  • Ability to work within a team and independently


Desirable

  • Able to act up into line manager role if required with support of the wider team.
  • Experience of working within heart failure
