An exciting opportunity has arisen for a highly motivated individual to join the Adult Integrated Care Dietetic team as a Band 7 Primary Care Network (PCN) Dietitian for the Hills, Brooks and Downs PCN (HBD) in Lambeth. The Adult Integrated Care Dietetic team are a friendly team who are proud and passionate about the work we do for our community dwelling patients. We have a dynamic and flexible approach to work. Main duties of the job

The post-holder will be a qualified autonomous clinical practitioner working with GP practices within HBD PCN who can assess, diagnose, treat, and manage dietetic problems and undifferentiated conditions.

The successful applicant will join our friendly and supportive community dietetics team. The post will cover a wide range of clinical conditions including management of nutrition support, Diabetes (type 2), weight management and lipid lowering.

Previous experience of working with a variety of nutritional needs is essential.

Job description

Job responsibilities

Assess, diagnose, triage, and manage patients, taking responsibility for the management of a caseload.

Develop and make use of your full scope of clinical practice, including skills relating to supplementary prescribing and investigation when trained to do so.

Support patients to set their own goals and be confident in their approach to self-manage their conditions where appropriate e.g. diabetes, high cholesterol & cardiovascular disease.

Provide care which is tailored to the individual needs.

Make prevention possible in primary care with dietetic interventions for conditions including but exclusive to: type 2 diabetes, lipid lowering and hypertension.

Work with the PCN to provide an equitable service which minimises health inequality

Liaises with PCN colleagues including commissioners, to ensure consistency of patient care and benefit to GP practices.

Supervise junior members of staff by taking part in 121 and appraisals

Person Specification



  • Computer literacy


  • Teaching on degree (or higher level) / professional course
  • Representative on academic / government committee / working party
  • Ability to analyse and Interpret statistics
  • Previous specialist related publications



  • Substantial clinical experience following graduation
  • Previous experience at a senior level
  • Experience of training student dietitians
  • Working as part of a team
  • Previous experience of conducting audits and / or research



  • BSc (Hons) Degree in Dietetics or equivalent
  • Current HCPC registration
  • Evidence of further postgraduate study in specialist area
  • Advanced BDA course in Nutritional Support e.g. PENG Course
  • Postgraduate training in cognitive behavioural therapy
  • Sound clinical knowledge
  • Evidence of CPD e.g. advanced diploma in dietetic practice or equivalent short courses
  • Sound knowledge of clinical issues relating to specialist area
