This is an exciting opportunity to join a dynamic and forward thinking team in acute respiratory medicine at Worcester Royal Hospital. The service covers a 31 bedded respiratory ward, acute respiratory admissions and inpatient respiratory referrals.

We are looking for a practitioner who has worked and gained relevant experience as a band 6 within respiratory medicine.

You will have excellent interpersonal skills and leadership qualities, you will have drive, vision and initiative to motivate others and deliver this service. Excellent decision making skills and evidence of continuous professional development are essential and you must also have experience of working in both respiratory and a multi-professional environment.

The successful applicant will receive specialist training in the assessment and management of patients with respiratory diseases as well as in-house training to support them in this role.

Main duties of the job

The Nurse Practitioner (NP) will receive specialist training under the supervision of Respiratory Consultants and an established Respiratory nurse practitioner team. It is expected that the NP will be an integral member of the respiratory team. They will participate predominantly in respiratory in-reach, assist with management of ward patients and respiratory referrals, requesting, recording and communication of investigations and respond to deteriorating patients. The successful candidate will have a particularly important role in identifying respiratory patients outlying from the respiratory wards and improving their access to high quality specialist respiratory care. It is expected that the successful candidate will provide support for Saturday Respiratory Consultant ward rounds at a frequency of 1 in 3 to 4.

Key working relationships:

Internal: Communicating with a varied range of people internal and external, clinical areas, Ward Managers, Senior Nurses, Head of Nursing, Resuscitation department, Medical Staff, Clinical Audit.

External: Agencies, GPs, Social workers, relatives, carers and patients.

Risk Assessment:

The post involves manual handling activities and exposure to body fluids and chemicals.

May involve some VDU work and individuals may be exposed to violence and aggression.

Job Purpose:

The role of the Nurse practitioner is dynamic and the post holder will be proactive in pioneering and developing the role in response to changes in practice and healthcare needs.

The post intends to improve patient outcomes though improving accessibility to services and improving quality.

The post holder is expected to exercise a high degree of professional autonomy and make critical decisions to satisfy the expectations and demands of the role.

Communication with a range of people internal and external

Dealing with stressful situations, for example breaking bad news.

Key Dimensions

Provide direct clinical care through health assessment, differential diagnosis, initiation of treatment, formulation and alteration of treatment and monitoring of care programmes, under the supervision of Consultant / ANP.

Facilitate development of clinical practice through the support of research and education of multidisciplinary team.

As a valued member of the MDT to contribute to the evaluation of care.

Become an expert clinical resource for other Health care providers.

Accept personal responsibility and accountability for infection prevention and control practice.

Ensure familiarity and comply with, all relevant policies.

Ensure mandatory training up to date.

Key Duties:

Specialist clinical skills

The NP will triage acute admissions and inpatient referrals for consultants and other members of the respiratory team to review and co-ordinate and assist with the Consultant ward round

The NP will make their own clinical review of patients when needed

The NP will navigate patients investigation pathways and support patients and staff on non respiratory wards with their specialist skills

The NP will work with the respiratory ward nurses and Matron to achieve safe patient selection for respiratory beds and maintain an audit of this process

The NP will contribute to data collection for service improvement in the acute respiratory service

Clinical/Professional Leadership

Ensure clinical practice reflects all current NMC documents including:

o Code of Professional Conduct

o Standards of record keeping

o Administration of medicines

o Scope of professional practice.

Organise, the clinical management of individual patients, utilising clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes where required. This will include monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations.

Respond to emergency situations utilising clinical judgement and decision making to initiate appropriate intervention and treatment for patients.

To work towards and achieve qualification and competence in Non Medical Prescribing.

Following attainment of a Non-Medical Prescribing qualification, to initiate drug therapy/medication within the parameters of agreed guidelines and in accordance with the current supply and prescribing legislation.

Monitor and evaluate outcomes using a patient centred approach, involving the patient and their carers where possible.

Utilise the role to maintain continuity for patients and their families.

Make and receive referrals from the multidisciplinary team.

Communicate patients condition, verbally and in writing, to other members of the health care team and contribute to the decision making process in patient management.

Develop appropriate advanced clinical and technological skills in response to the changing needs of the service.

Participate in respiratory In reach, ward rounds, perform case presentations and contribute to the decision making process in terms of patient management.

Take every opportunity to promote the health of the patients and initiate health education strategies as appropriate.

Participate in risk assessments and risk management in collaboration with the multidisciplinary team.

Utilise quality and research data to develop best practice within the trust.

Assist in evaluating outcomes of clinical practice, taking into account guidance provided through benchmarking and clinical governance.

Participate in the development of clinical protocols in response to advances in the role of the nurse practitioner.

Be actively involved in networking at all levels within the trust

Contribute to the directorate development plan

Be aware of the Trust complaints policy.

Person Specification

Qualifications

Essential

  • - First level NMC registration
  • - Related post-basic training
  • - Recognised NMC Teaching and Assessing or Mentorship qualification
  • - First Degree
  • - Current ILS/ALS provider


Desirable

  • - Working towards or willingness to undertake Masters level study.
  • - Recognised Health Assessment course.


Experience

Essential

  • - Respiratory ward experience at Band 6 level.
  • - Experience of supporting change/project management.
  • - Involvement in Evidence Based Healthcare agenda.
  • - Risk management.
  • - Experience of patients requiring acute and domiciliary Non-Invasive Ventilation.
  • - Experience of chest drain and pleural catheter management
