Band 6 Registered Professional for the Mental Health Access Hub and Home Treatment Team

Based at Avenue House, Nuneaton, working across the North Warwickshire and Rugby geographical area

There have been new exciting changes made to access and referrals into Secondary Care mental health services.

We are currently looking for motivated and enthusiastic Band 6 Registered Professionals (Mental health nurses, social workers and OTs) to join our team based in Nuneaton.

Our Mental Health Access Hubs and Home Treatment Teams, which have moved into triage and assessment teams with a separate team for home treatment form part of the community provision for secondary mental health care in Coventry and Warwickshire.

The team will work with individuals alongside other teams and MDTs to determine the required least restrictive interventions to facilitate support, recovery and crisis management.

You will need to demonstrate experience of working with clients with mental health problems or experiencing mental health crisis. You will need to demonstrate strong assessment skills, be willing to take positive risks and able to plan care and implement interventions. You must be committed to working with people with complex needs, ensuring that multi-disciplinary meetings are set up to review the patients health and social needs, to support people in the community as an alternative to hospital admission. The service is an age independent multidisciplinary team. We work with clients 16 plus providing a high level of care.The Service covers a large geographical area and the post holder will need to be confident about lone working and working as an autonomous clinician. There is a shared multidisciplinary approach to planning care. There is a strong learning culture within the team and the successful applicant would be expected to contribute to this.

The successful candidate must have excellent communication skills, demonstrate a caring, compassionate and professional attitude to the client group and have evidence of working in an acute psychiatric setting. They will also need to enjoy working in a fast paced team. They will demonstrate a sound knowledge of the acute pathway and be involved in supporting and planning early discharge and admission avoidance

Person Specification QUALFICATIONS Essential
  • Relevant Professional Qualification i.e RMN, DipCOT
  • Expertise within a discipline underpinned by theory
  • Professional clinical knowledge acquired through degree/ diploma
KNOWLEDGE & SKILLS Essential
  • Excellent written communication skills
EXPERIENCE Essential
  • Experience in Acute Mental health care setting
  • Experience of undertaking assessment, formulation, risk assessment and positive risk management
  • Evidence of continuing professional development
Other Essential
  • Must have full driving licence and access to a motor vehicle for business use
