An exciting opportunity has arisen for an experienced nurse, RGN, to join a dynamic team of nurses within CWP that provide commissioning services for people with complex care needs who are eligible for NHS Continuing Healthcare funding or NHS-funded nursing care.

The candidates should have experience of undertaking CHC assessments, working with patients, families and carers, and have a sound knowledge of the NHS Framework for Continuing Healthcare. The successful candidates will be responsible for the assessment process from referral, to assessment, review, care commissioning and case management for individuals who are eligible for CHC.

Whilst travel was limited during the Covid-19 pandemic and much work has been done virtually this role ordinarily involves a considerable amount of travelling across the North West therefore a valid driving licence and access to a vehicle for work purposes is essential.

Main duties of the job

Full details are available on the JD / Person specification but in summary the successful candidate will need to :
  • Ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the CCG within the service's key performance indicators
  • Complete all review assessments in line with the service's key performance indicators
  • Work closely with acute hospitals to support discharge to assess pathways
  • Accurately input information onto departmental databases and tools used in Continuing Healthcare adhering to NMC standards for record keeping
  • Assist and support individuals and representatives in relation to the Continuing Healthcare process
  • Commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
  • Case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care
  • Provide assurance regarding quality of individual packages of care by undertaking reviews as required and reporting quality/safeguarding concerns through the agreed reporting route in a timely manner;
  • Undertake urgent reviews of individuals where quality and safety concerns have been identified.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Person Specification Qualifications Essential
  • Degree or equivalent level of experience in healthcare
  • Evidence of professional development NMC registration - RGN
  • Relevant post basic academic/professional
  • Two years post registration qualified
Knowledge and Expertise Essential
  • Excellent knowledge around NHS Framework for Continuing Healthcare
  • Demonstrable relative experience and knowledge of current key policies and priorities.
  • Understanding of service provision for the client group.
  • Sound knowledge and understanding of the current NHS reforms.
  • Understanding of relevant, current NHS and social care market mechanisms
  • Excellent written and verbal communication information
  • Understanding of the commissioning agenda including its scope, structure, methods of operation, processes and interrelationships
