An exciting opportunity has arisen within our Integrated Care Division for a ward manager to provide clinical and operational skills to support the delivery of our multi-speciality virtual ward

Virtual ward is a safe and efficient alternative to NHS bedded care. Virtual wards support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home. This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital. The delivery of digitally enabled care (virtual care) including virtual wards is a national priority embedded within current health and social care policy.

Main duties of the job

The Virtual Ward Sister/Charge Nurse is responsible for the ambitious delivery and further development and management of the Virtual Ward within Herefordshire.

The successful applicant willwork with all members of the Virtual Ward clinical team within the multi-disciplinary framework. You will maintain a high standard of patient care and lead on the management of all patients within the Virtual ward. You will have a key role in identifying suitable patients to move from acute wards into the Virtual ward ensuring that this transfer is completed efficiently and safely.

You will work closely with key stakeholders including acute wards and hospital services, medical teams, community services and primary care to ensure the safe and effective management of all patients within the virtual ward

Person Specification

Education & Qualifications

Essential

  • Registered Nurse
  • Post Registration Qualification within Specialty or equivalent
  • Relevant 1st degree or equivalent, or working towards
  • A1 award or equivalent


Skills, Knowledge & Abilities

Essential

  • Leadership ability
  • Time Management of self & others
  • Demonstrate an ability to problem solve highly complex situations
  • Motivational, negotiating/persuasive skills


Desirable

  • Writing Service Development Proposals


Experience

Essential

  • Supporting Evidence of Post Registration Education within sphere of practice
  • Demonstrate an ability to deliver clinical skills in practice
  • To provide evidence of involvement of service development
  • To demonstrate awareness of the impact of changing services
