Band 5
Contract Type
Part Time
Southern Health Foundation Trust has an exciting opportunity for a Band 5 Registered General Nurse to join the Community Assessment Unit at Petersfield Hospital and work with the Urgent Care Response (UCR) team in the community. The post is a 30-hour-per-week substantive post covering the Southeast Hampshire area.

You will be working within the clinic setting at Petersfield Hospital and in the community setting where you will be responding to the urgent care needs of service users in their own homes.

This is an excellent opportunity if you enjoy having varied work experience and want to broaden your knowledge and skills in both a clinic and community setting. This role is critical to preventing hospital admissions as part of the Ageing Well agenda and monitoring patients in their own home as part of the UCR virtual ward.

Ideally the successful candidate will possess excellent communication skills and be competent in linking with the larger multidisciplinary team.

This position involves a proportionate amount of time driving when carrying out UCR visits, therefore you will need to be a car owner and have access to a vehicle for work with the correct level of insurance.

The transformation will happen in stages Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning across in May 2024. Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
