An exciting opportunity has arisen within the Integrated Medicine and Patient Flow (IMPF) Division as we look to expand our Non-Medical Consultant workforce in line with the exciting grand opening of the Acute Frailty Assessment Unit in March 2024. A business case to expand our footprint as the Acute Frailty Unit was agreed in 2023 and is in its implementation phase in readiness for opening in March.

As a Non-Medical Consultant you will work clinically in conjunction with the medical workforce. You will be a registered nurse or Allied Healthcare Professional with substantial experience.

You will be instrumental in developing our Acute Frailty Service.

  • Have a highly visible clinical profile, providing both advisory and clinical input into patient care, spending a minimum of 50% of time in clinical practice. The post holder will have clinical responsibility for a patient group within Frailty services.
  • Autonomously act and develop recognition for their expertise in the field of frailty and long term condition management having developed specialist clinical knowledge.
  • Ensure patient / client centred care and family and carer collaboration and where appropriate promote maintenance of health, wellbeing and independence.
  • Apply advanced theoretical, evidence based and practical knowledge to the clinical care of patients presenting to the Frailty pathway when delivering and shaping services.
  • Work collaboratively with multidisciplinary teams to optimise clearly defined patient outcomes.
  • Manage a defined caseload that will focus on frailty patient presentations: Receive referrals, assess, order, interpret and act upon investigative tests, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients within the Trust.
  • Provide leadership, guidance and exercise judgement in making challenging patient centred decisions within current best practice guidance, relevant legal frameworks and with clear understanding of the relevant professional and applied ethical frameworks.

Qualifications

Essential

  • Registered Nurse or AHP NMC/HCPC Registered
  • MSc in Advanced Practice or completion of NHSE Accredited e-portfolio
  • Independent Non- Medical Prescriber
  • Speciality Qualification
  • Teaching Qualification
  • Active Professional portfolio in specialty
  • Leadership qualification


Desirable

  • Working towards or relevant Doctorate


Experience

Essential

  • Experience of clinically leading a service
  • Experience of manging patients ? with chronic and acute presentations who are frail and have long term conditions
  • Significant evidence of delivering teaching programmes to a variation of staff groups
  • Minimum of 3 years experience of working autonomously as an Advanced Practitioner (Band 8a) within the speciality (frailty and/or long term conditions)
  • ignificant experience of holding clinical responsibility for a whole patient pathway
  • ignificant experience of planning, assessing and directing care provided to patients across a whole pathway.
  • Experience of service development and redesign
  • Evidence of involvement in local & national networks
  • Significant experience of training and developing staff
  • The post holder must have regular access to transport to meet the demands of the role
  • Evidence of leading audit/research


Desirable

  • Presentation of papers and publications in peer reviewed journals
  • Lead/chair national networks relating to speciality

  • Evidence of excellent autonomous clinical practice including independent decision making and problem solving
