If this is the case we would like to meet you. We are seeking to recruit a Band 6 Nurse to join our existing team on Marham Ward at The Queen Elizabeth Hospital in King's Lynn, Norfolk.

We are seeking an enthusiastic, flexible and motivated Nurses to work within our progressive team. This role will provide specialist expert advice to support and facilitate high quality individualised care to patients, relatives and carers.

Main duties of the job

  • Communicate with patients, relatives, carers and members of the multidisciplinary team - by telephone, face to face, in writing or electronically to ensure that discharge takes place in a timely fashion.
  • Act as a resource to ward staff offering advice and help on all aspects of discharge planning, prioritising complex cases.
  • Develop and maintain effective relationships with other agencies involved in discharge ensuring they are involved at the earliest opportunity in the discharge of patients with complex needs.
  • Attend case conferences as appropriate, giving advice / input as required.
  • Attend daily progress meetings with Social Service and PCT discharge team representatives.
  • Maintain accurate records in relation to individual patient discharge.
  • Ensure appropriate information is available to patients, relatives and carers in order for them to understand the discharge planning process.
  • Contribute to effective communication and liaison within Discharge Planning Team supporting a unified approach to service development.
  • Act as an advocate for patients when appropriate in ensuring their discharges are safe and effective.
  • Respect confidentiality regarding all information received relating to patients, employment, other members of staff and the general public. Uphold the requirements of the Data Protection Act.

Person Specification



  • RGN Degree/Diploma in nursing
  • Evidence of Post Registration Educational Continuing development


  • Recognised NMC Teaching and Assessing Qualification or equivalent, or willingness to undertake, or in progress.



  • Post registration experience
  • Evidence of innovation and achievements in clinical practice
  • Can demonstrate the use of appropriate criteria to prioritise patient case load
  • Can demonstrate experience of difficult conversations.


  • Evidence of working as part of a multi- disciplinary working experience
  • Demonstrates experience of clinical risk management
  • Community experience

Skills, abilities, and knowledge


  • Proven compassionate care experience.
  • Demonstrates understanding of efficient and effective use of resources where the workload can be unpredictable
  • Ability to prioritise
  • Demonstrates a high level of clinical expertise
  • Can demonstrate knowledge of current discharge to assess processes.
  • Knowledge of NHS plan and national nursing strategy
  • Demonstrate knowledge of the Mental Capacity Act and its implications for practice


  • Good understanding of clinical governance issues especially risk management and audit procedures, including evaluation
  • Audit experience / research skills
  • Experience of Patient and Public Involvement in Service Delivery
